Definition

Potassium is the body's main intracellular cation, essential for nerve signaling and for skeletal, smooth, and cardiac muscle contraction.[1] The kidneys regulate it, with aldosterone controlling how much gets secreted into urine.[2] On a lab panel it's measured as serum potassium, and it's also a dietary nutrient logged from food.[3]

What Potassium Measures

Most of the body's potassium sits inside cells rather than in the blood, which is what makes it the primary intracellular cation.[1] That location gives potassium an outsized job: it activates enzymatic reactions and plays a direct role in nerve impulse transmission, in the contraction of skeletal and smooth muscle, and in the electrical activity that keeps cardiac muscle beating on rhythm.[1] The sodium-potassium pump is what maintains the steep gradient between the potassium-rich inside of a cell and the potassium-poor fluid outside it, and the kidneys handle the rest, reabsorbing potassium early in the nephron and adjusting secretion further along under aldosterone's control.[2]

How It's Measured

Serum potassium comes from a standard blood draw and is reported in milliequivalents per liter. It's one of the eight values on a basic metabolic panel and one of the electrolytes on an electrolyte panel, ordered as a set with sodium, chloride, and bicarbonate rather than on its own.[3][4] Dietary potassium is tracked a different way entirely, as milligrams totaled from the foods and drinks you log rather than measured in blood.

What the Research Shows

Potassium's role as the primary intracellular cation, and its specific involvement in nerve transmission and muscle contraction across skeletal, smooth, and cardiac tissue, is documented in the clinical reference literature.[1]

The StatPearls review of electrolyte physiology describes the sodium-potassium ATPase pump as the mechanism maintaining the sodium-potassium gradient across cell membranes, with the kidneys reabsorbing potassium in the proximal tubule and thick ascending limb, and aldosterone increasing secretion in the distal tubule.[2]

A 2006 study in the Journal of Clinical Endocrinology & Metabolism tied progesterone directly to aldosterone across the menstrual cycle, finding a correlation with urinary aldosterone, basal serum aldosterone, and the aldosterone response to infused angiotensin II that didn't hold for estradiol.[5] Since aldosterone is what drives potassium secretion in the kidney, that cyclical shift extends to how potassium is handled as well as sodium.
