Hypothyroid myopathy, muscle weakness tied to low thyroid hormone, is described as affecting an estimated 30% to 80% of people with hypothyroidism, most often the proximal muscles of the shoulders, hips, thighs, and neck.[2] Thyroid hormone is a primary regulator of mitochondrial function in skeletal muscle, and its deficiency is linked to selective atrophy of fast-twitch fibers and disrupted glycogen and oxidative metabolism, which together are the proposed mechanism behind the weakness.[2] The same research describes muscle strength typically improving within two to three weeks of restoring thyroid hormone to normal levels, though full resolution can take longer.[2]

Vitamin D is linked to muscle weakness through a different pathway. Severe, prolonged deficiency triggers a secondary rise in parathyroid hormone, and research describes muscle twitching and weakness, alongside bone pain and fatigue, as a feature of that more pronounced deficiency, while most people with vitamin D deficiency have no symptoms at all.[3]
