What Chloride Measures

Chloride is an anion found predominantly in extracellular fluid, the fluid that surrounds rather than fills your cells.[1] Extracellular fluid runs high in sodium, chloride, and bicarbonate together, while the fluid inside cells runs the opposite way, higher in potassium, phosphate, and magnesium.[2] Because chloride tends to follow sodium, the two anions and cations move together across cell membranes to keep fluid distributed and the body's overall ionic charge close to neutral.[2] The kidneys are the main regulator of serum chloride, reabsorbing most of what's filtered through a mix of active and passive transport in the tubules.[1]

Chloride has a second job that has nothing to do with fluid balance. In the stomach, specialized cells move chloride through dedicated channels into the gastric lumen, where it combines with hydrogen ions to form hydrochloric acid, the acid that helps digest food.[4]
