Definition

Sodium is the major cation in extracellular fluid, where it helps set blood volume and supports nerve and muscle signaling.[1] The kidneys keep sodium levels in tight balance, largely through the hormone aldosterone.[1] On a lab panel it's measured as serum sodium, and it's also a dietary nutrient logged from food.[2]

What Sodium Measures

Sodium is one of the essential electrolytes in the fluid that surrounds your cells, where it's the main ion responsible for holding that fluid's volume steady.[1] Because water follows sodium across cell membranes, sodium is also central to how much fluid stays in your bloodstream versus your tissues, and to the electrical gradient nerve and muscle cells use to fire.[1] The kidneys are the main regulator. Most sodium reabsorption happens in the proximal tubule, and the hormone aldosterone fine-tunes how much gets reclaimed further along, through sodium-chloride transport in the kidney.[1]

How It's Measured

Serum sodium comes from a standard blood draw and is reported in milliequivalents per liter. It's rarely ordered by itself, since it's one of the eight values on a basic metabolic panel and one of the electrolytes on an electrolyte panel, tests ordered together specifically so fluid balance and kidney function can be read as a set.[2][3] Sodium is also tracked a completely different way: as milligrams consumed, added up from the foods and drinks you log rather than measured in blood.

What the Research Shows

Sodium's role in extracellular volume and the kidney's aldosterone-driven control of it are established physiology, and they're the basis for reading serum sodium alongside potassium, chloride, and bicarbonate as a set.[1]

A 2006 study in the Journal of Clinical Endocrinology & Metabolism measured aldosterone and progesterone together across the menstrual cycle and found progesterone directly correlated with urinary aldosterone, basal serum aldosterone, and the aldosterone response to infused angiotensin II, independent of the renin-angiotensin system.[4]

Pregnancy shows sodium's role at a larger scale. A 2017 review in the American Journal of Clinical Nutrition describes a 30 to 50 percent expansion of plasma volume by late pregnancy, driven substantially by increased sodium and water reabsorption in the kidney.[5]
