What Bicarbonate Measures
Cellular metabolism constantly produces carbon dioxide, which combines with water to form carbonic acid, and carbonic acid then splits into bicarbonate and a hydrogen ion, a reaction sped up by the enzyme carbonic anhydrase.[1] That equilibrium is one of several buffer systems in the body, and it works by shifting toward whichever side of the reaction the current pH calls for, soaking up excess acid or releasing more of it as needed.[1] Most of the carbon dioxide carried in blood, roughly 80 percent, travels in this bicarbonate form rather than as dissolved gas or bound to hemoglobin, which is why a blood test for "CO2" is really reading bicarbonate.[2]
Two organs keep the buffer in balance on different clocks. The lungs adjust how much CO2 gets exhaled, which can shift blood pH within minutes to hours. The kidneys work over days, adjusting how much bicarbonate they reabsorb versus how many fixed acids they excrete.[1]
