Definition
Cortisol is a steroid hormone made in the adrenal cortex and controlled by the HPA axis: the hypothalamus releases CRH, the pituitary releases ACTH, and the adrenal glands respond with cortisol. It helps regulate blood glucose, metabolism, and immune function, and follows a strong daily rhythm that peaks around waking.[1]

What Cortisol Measures
Cortisol is the main output of the HPA axis, the signaling chain that runs from the hypothalamus through the pituitary to the adrenal glands. It raises blood glucose by driving glucose production in the liver, shapes how the body handles protein and fat, and helps regulate the immune and inflammatory response.[1] It's often called the stress hormone, but it's working around the clock, stress or not.
The clock part matters most for tracking. Cortisol starts climbing in the last hours of sleep, peaks around the time you wake up, then falls through the day to a low point at night.[1] On top of that curve sits the cortisol awakening response, a marked rise over the first 30 to 45 minutes after waking that researchers treat as its own measure.[2]
How It's Measured
Labs measure cortisol in three sample types, and they don't measure quite the same thing. A serum blood draw captures total cortisol, most of which rides on carrier proteins with only a small unbound fraction biologically active. Saliva reflects that free, unbound fraction, which is why awakening-response studies use spit samples collected at home. A 24-hour urine collection sums the free cortisol excreted over a full day instead of catching one moment.[3] Different assays also disagree with each other, so results from different labs and methods aren't directly interchangeable.[3]
Add the daily rhythm and single results get hard to compare. A morning draw and an afternoon draw from the same person on the same day will differ simply because of the clock. That's why draw time gets recorded alongside the result.
Why People Track Cortisol
Cortisol shows up on many hormone panels, including the at-home kits that test saliva or urine, so plenty of people end up with cortisol results whether they went looking or not. It also connects to cycle biology: the HPA axis and the reproductive hormone axis interact, and researchers study how cortisol behaves across the menstrual cycle.[4]
One thing no tracker can offer: cortisol from a wrist. No consumer wearable measures cortisol. Watches and rings track heart rate, HRV, temperature, and movement, and some apps label combinations of those as stress scores, but cortisol itself only shows up in a lab sample. What the Research Shows
Baseline cortisol runs modestly higher in the follicular phase than in the luteal phase. A meta-analysis of 35 studies found the difference with a small effect size, and a 2022 meta-analysis of 121 longitudinal studies in 2,641 healthy cycling women confirmed the same direction, again small, with comparisons between more precise phases mostly insignificant.[4][5]
Reactivity is where the cycle shows up more clearly. A 2023 meta-analysis found higher cortisol reactivity to acute stressors in the luteal phase than the follicular, meaning cortisol rises more in response to the same challenge. The authors also flagged how thin the evidence is: only three studies had data solid enough to pool.[6]
Individual differences are large across all of this work. The phase effects are population averages with small effect sizes, so they describe groups, not any one person's pattern.[5]
