Definition
Heart rate variability (HRV) is the variation in time between consecutive heartbeats, measured in milliseconds. It reflects the ongoing push and pull between your sympathetic and parasympathetic nervous systems, the two halves of the autonomic nervous system that speed up or slow down your heart rate in response to stress, activity, and rest.[1]

What HRV Measures
A perfectly steady heartbeat isn't actually the goal: some variation between beats is a sign that your nervous system can adjust quickly to changing demands. HRV is most often reported as SDNN, the standard deviation of the time between beats calculated over a long window, generally hours rather than minutes. A separate calculation, RMSSD, measures beat-to-beat variation from a short reading and reflects parasympathetic ("rest and digest") activity more specifically. SDNN and RMSSD use different math on different time windows and land on different scales, so they aren't interchangeable. RMSSD gets its own entry in this encyclopedia.[1]
How It's Measured
Consumer wearables estimate HRV using photoplethysmography (PPG), an optical sensor that shines light into the skin and detects the tiny changes in blood volume with each heartbeat, most often worn at the wrist. Most devices calculate the night's SDNN from beat-to-beat intervals captured while you sleep, when the body is still and the signal is cleanest. A 2025 validation study put five consumer wearables against a chest-strap ECG reference and found accuracy varies by device: some tracked the reference closely overnight, others less so.[2]
Why People Track HRV
The same autonomic nervous system that produces HRV also handles stress and recovery, so people track HRV as a broad marker of both, watched as a trend over days and weeks rather than read as a single night's number. It also shifts across the menstrual cycle, generally running lower in the luteal phase, the second half of the cycle after ovulation.

What the Research Shows
A 2019 meta-analysis pooling 37 studies and more than 1,000 individuals found a significant, moderate decrease in cardiac vagal activity (a category of HRV measures) from the follicular phase to the luteal phase of the menstrual cycle, with the sharpest drop-off heading into the premenstrual days.[3]

Is HRV the same as heart rate?
No. Heart rate is how many times your heart beats per minute. HRV is the variation in the time gap between those beats, measured in milliseconds. A slow heart rate and a fast heart rate can each come with high or low HRV. They describe different things about your cardiovascular system.
What's the difference between HRV (SDNN) and HRV RMSSD?
Most wearables report HRV as SDNN, the standard deviation of beat-to-beat intervals calculated over hours, usually overnight. RMSSD is a separate calculation, typically taken from a short on-demand reading like a camera pulse check, on a different scale and time window. The two numbers aren't meant to be compared directly.
