Definition

DHEA-S (dehydroepiandrosterone sulfate) is the sulfated form of DHEA, an androgen precursor made mainly by the adrenal glands.[1] It circulates at higher concentrations than any other adrenal steroid, and peripheral tissues convert it into active androgens and estrogens.[2] Labs measure it from a blood draw, usually in micrograms per deciliter (µg/dL).[4]

What DHEA-S Measures

Your adrenal glands make DHEA in the zona reticularis, the innermost layer of the adrenal cortex.[1] An adrenal enzyme then attaches a sulfate group, and the result is DHEA-S.[2] That sulfate group changes how the hormone behaves in blood. DHEA-S clears slowly, so it builds up to concentrations around 300 times higher than free DHEA.[2] On its own it has little hormonal activity. It works as a reservoir: tissues throughout the body take up the precursor and convert it into active androgens like testosterone, and from there into estrogens.[1],[2] A DHEA-S result describes the size of that circulating adrenal precursor pool, not the activity of any single downstream hormone.

How It's Measured

DHEA-S comes from a standard blood draw, usually reported in micrograms per deciliter (µg/dL) and often run alongside other sex hormone tests.[4] Timing matters less than it does for many hormones. Cortisol follows a daily rhythm, peaking near waking and falling through the day.[5] DHEA moves on a similar daily pattern.[2] DHEA-S doesn't. Its slow clearance keeps the level steady across the day, so a morning draw and an afternoon draw read close to the same.[1],[2] That steadiness, along with its long half-life in blood, is why lab panels usually report DHEA-S rather than DHEA.[2] Some at-home urine hormone panels report a DHEA-S value too.

Why People Track DHEA-S

DHEA-S usually appears as one line on an androgen panel, next to total testosterone, free testosterone, and SHBG. Androgens in women come from two sources, the ovaries and the adrenal glands, and DHEA-S is the marker that speaks for the adrenal side.[2] So when a clinician orders a hormone workup, DHEA-S fills in the adrenal part of the androgen picture. It's a descriptive part of that panel, not something to interpret on your own. The clinician reading the full workup draws the conclusions.

What the Research Shows

In a cross-sectional study of 981 men and 481 women aged 11 to 89, serum DHEA-S peaked at ages 20 to 24 in men and 15 to 19 in women, then declined steadily with age.[3] By age 70, average levels had fallen to a fraction of the young-adult peak.[3] That's a population-wide curve, not a rate that applies evenly to any one person.

Endocrinology references describe the same arc: DHEA-S rises in childhood during adrenarche, peaks in the third decade of life, then falls by roughly 2 to 5% per year, a pattern sometimes called adrenopause.[1]

The same cohort study also found a consistent sex difference, with men showing significantly higher DHEA-S than women across ages 20 to 69.[3]

What's the difference between DHEA and DHEA-S?

DHEA-S is DHEA with a sulfate group attached. The sulfated form clears from blood far more slowly, so it circulates at much higher and steadier concentrations than free DHEA. That's why lab panels usually report DHEA-S rather than DHEA.

Does the time of day matter for a DHEA-S measurement?

At the population level, DHEA-S holds steady across the day. Unlike cortisol, which peaks near waking and falls through the day, DHEA-S shows no daily rhythm because it clears from blood slowly. That steadiness is one reason labs prefer it over DHEA.

Is DHEA-S the same as the DHEA supplement?
