Definition

Saliva hormone testing measures a hormone in a spit sample instead of blood. For salivary cortisol, that method has real clinical backing, including its use as a first-line test for Cushing's syndrome.[2] For salivary progesterone, testosterone, and estradiol, major medical societies say saliva testing shouldn't be used to guide hormone therapy dosing.[3]

What Saliva Hormone Testing Measures

A saliva hormone test reports how much of a hormone shows up in oral fluid rather than blood. Steroid hormones like cortisol, progesterone, testosterone, and estradiol travel through the bloodstream mostly bound to carrier proteins, and only the small unbound fraction can leave capillaries and diffuse into saliva.[1] That unbound fraction isn't the same size for every hormone. Less than 5% of circulating steroid hormone is free at any given time, estrogen shows up in saliva at levels low enough to make it methodologically hard to measure, and progesterone is present in saliva at higher levels but doesn't necessarily reflect what's happening in tissue.[3]

How It's Measured

Salivary hormone tests use a passive-drool or swab sample, usually timed to a specific point in the day or cycle because levels shift with the clock or with cycle phase. Labs run the sample through immunoassay or, for more precision, liquid chromatography-tandem mass spectrometry. Timing matters most for cortisol: the guideline used to evaluate Cushing's syndrome calls for two late-night samples, collected at the point in the daily rhythm when cortisol should be at its lowest.[2] There's currently no FDA-approved salivary or urinary test for any steroid hormone.[3]

Why People Track Saliva Hormone Results

Salivary kits get marketed as a needle-free way to check hormone levels at home, and direct-to-consumer panels commonly bundle cortisol with progesterone, testosterone, and estradiol under labels like "hormone balance" or "stress and hormone" panels. That's often how people end up with all four results at once, whether or not each one is telling them something a clinician would act on. What the Research Shows

Late-night salivary cortisol is one of the few salivary hormone tests with real backing. An Endocrine Society clinical practice guideline lists it as a first-line test for diagnosing Cushing's syndrome, alongside 24-hour urine free cortisol and dexamethasone suppression testing, with reported sensitivity and specificity above 90% when the sample is run in an experienced lab.[2] The same guideline flags real caveats: tobacco use and licorice can produce false positives, and the test isn't considered appropriate for shift workers or people without a stable sleep schedule.[2]

The other three analytes don't have that kind of backing. An Endocrine Society scientific statement on compounded bioidentical hormones found "no evidence that monitoring compounded HT with serial salivary or blood testing is effective, except in the case of thyroid hormone."[4] The North American Menopause Society's 2022 position statement goes further, stating plainly that "salivary and urine hormone testing to determine dosing are unreliable and not recommended."[5]

Part of the problem sits in the biology itself. A 2023 clinical consensus from the American College of Obstetricians and Gynecologists explains that estrogen shows up in saliva at levels low enough to make it methodologically hard to measure, and while progesterone is present in saliva at higher levels, what shows up there doesn't necessarily reflect what's happening in tissue.[3]

For one specific use, it's well validated. Why does the evidence differ so much between the four hormones?

The three sex steroids don't have an equivalent validated target. Estrogen shows up in saliva at levels low enough to be methodologically hard to measure, and salivary progesterone doesn't necessarily reflect what's happening in tissue elsewhere in the body.
