Definition

What Caffeine Does

Caffeine is the most widely used psychoactive stimulant in the world, sourced mainly from coffee beans but also occurring naturally in tea, cacao beans, kola nuts, yerba mate, and the guarana berry, and added to many sodas and energy drinks.[1] It acts by antagonizing all four adenosine receptor subtypes, and the antagonism of the A2a receptor specifically is what's responsible for the wakefulness effect people feel from it.[1]

How long caffeine sticks around varies a lot by who's taking it. In an average adult, the half-life is roughly 5 hours, but it's notably longer in some groups and shorter in others. Smokers clear caffeine up to 50% faster than nonsmokers, while pregnant people, especially in the third trimester, can have a half-life prolonged to as long as 15 hours.[1]

Chronic use of low-dose estrogen-containing oral contraceptives has been shown to meaningfully slow caffeine clearance: one pharmacokinetic study found an average half-life of 7.9 hours in oral contraceptive users versus 5.4 hours in matched non-users.[4] The natural menstrual cycle is a murkier picture. A study that measured CYP1A2-dependent caffeine metabolism directly across the cycle found no significant difference in clearance between the follicular and luteal phases, concluding that dose adjustment by cycle phase wasn't warranted based on that pharmacokinetic marker.[3] Research on this specific question is genuinely mixed rather than settled.

What the Research Shows

ACOG's Committee Opinion No. 462 describes moderate caffeine consumption, defined there as less than 200 mg per day, as not appearing to be a major contributing factor in miscarriage or preterm birth, based on the evidence available to the committee. The opinion states plainly that caffeine's relationship to fetal growth restriction remains undetermined.[2] That's a description of a professional body's published position, not a recommendation from Go Go Gaia.

The extended half-life seen in pregnancy has a documented mechanism behind it: caffeine crosses the placenta, and pregnancy, especially in the third trimester, is one of the conditions StatPearls lists as prolonging caffeine's half-life well beyond the roughly 5-hour adult average.[1]

On hormonal contraceptives, the pharmacokinetic study found that oral contraceptive users had both a longer half-life (7.9 vs. 5.4 hours) and a markedly slower plasma clearance rate (1.05 vs. 1.75 mL/min/kg) than non-users, pointing to impaired activity of the liver enzyme that breaks caffeine down.[4] That's a different, more consistent finding than the natural-cycle research, where at least one directly measured study found no phase effect at all.[3]
