Definition
A tremor is an involuntary, rhythmic, oscillating movement of a body part, most often the hands.[1] Everyone has a fine, fast, low-amplitude physiologic tremor at rest, and it's the baseline that certain triggers, and certain distinct conditions, make more visible.[1]

Physiologic tremor is a high-frequency (10 to 12 Hz), bilateral, symmetric action tremor tied to a known, often temporary cause, such as a medication, low blood sugar, or hyperthyroidism.[1] That's different from a pathologic tremor like essential tremor, a distinct neurologic condition with its own diagnostic pattern rather than an exaggerated version of the normal baseline.[1]

What the Research Shows
Hyperthyroidism affects an estimated 1.3% of the US population, with the 1977 Whickham Survey finding it roughly 10 times more common in women than men (2.7% versus 0.23%).[2] Tremor is described as part of the hyperadrenergic, hypermetabolic state driven by excess thyroid hormone, alongside a wider set of symptoms tied to that same mechanism.[2]
Caffeine's link to tremor is the textbook teaching, physiologic tremor is described as aggravated by caffeine in contrast to essential tremor, which usually isn't.[1] But the controlled evidence is thinner than that reputation suggests. A double-masked, placebo-controlled study of 17 surgeons found caffeine raised hand tremor amplitude by 31% versus baseline, but that difference wasn't statistically significant against placebo.[3] An earlier study found a single 325 mg dose of caffeine didn't measurably increase physiologic, essential, or parkinsonian tremor at one, two, or three hours after ingestion, even though some participants believed coffee made their hands shakier.[4]
