Definition
Omega-3 and omega-6 fatty acids are two families of polyunsaturated fat, tracked separately in your food logger. The main omega-3s are ALA, EPA, and DHA, and the main dietary omega-6 is linoleic acid.[1] Both are essential fatty acids, meaning the body can't build them and depends on food to supply them.[1]

What Omega-3 and Omega-6 Are
Omega-3s and omega-6s are both polyunsaturated fatty acids, distinguished by where their first carbon-carbon double bond sits counting from the methyl end of the chain: three carbons in for omega-3, six for omega-6.[1] Most of the research on omega-3 focuses on three specific ones: alpha-linolenic acid (ALA), eicosapentaenoic acid (EPA), and docosahexaenoic acid (DHA). ALA is a shorter, 18-carbon chain found in plant oils like flaxseed, soybean, and canola, while EPA and DHA are longer-chain omega-3s found mainly in fish, shellfish, and algae.[1] The body can convert some ALA into EPA and then DHA, but that conversion happens mostly in the liver and is inefficient, with reported rates under 15%, which is why getting EPA and DHA directly from food or supplements is the more reliable way to raise those two specifically.[1]
Linoleic acid is the major dietary omega-6, found widely in vegetable oils, nuts, and seeds, and arachidonic acid is another omega-6 the body makes from it.[1] Both fatty acid families get used to build eicosanoids, signaling molecules involved in the body's cardiovascular, pulmonary, immune, and endocrine systems, and the two families compete for some of the same enzymes in that process.[1] Omega-3s in particular inhibit the enzyme that produces inflammatory eicosanoids from arachidonic acid, and also feed a separate pathway that produces its own inflammation-resolving molecules, which is the mechanism most often cited behind omega-3's anti-inflammatory reputation.[3]

Why People Track Omega-3 and Omega-6
Omega-3 is one of the most talked-about nutrients in wellness content, often framed around a target ratio of omega-6 to omega-3 intake. The guideline-grade picture is narrower than that framing suggests. Some researchers have proposed that the relative intakes of omega-6s and omega-3s matter for chronic disease risk, but NIH's own fact sheet is direct about the state of the evidence: the optimal ratio, if any, hasn't been defined, and other researchers have concluded that such ratios are too non-specific to be useful, since raising EPA and DHA levels appears to matter more than lowering linoleic acid or arachidonic acid.[1]
One area with a more specific, cycle-relevant evidence base is menstrual pain. A 2022 systematic review and meta-analysis of randomized controlled trials found that omega-3 polyunsaturated fatty acid supplementation reduced the severity of primary dysmenorrhea. What the Research Shows
Cardiovascular research on omega-3, primarily EPA and DHA, has produced a mixed record. Large randomized primary-prevention trials have had inconsistent results: VITAL, which tested 1 g/day of omega-3 in over 25,000 adults, didn't significantly reduce combined major cardiovascular events but did find a statistically significant 28% reduction in total heart attack rates, while ASCEND, a similar trial in adults with diabetes, found no significant effect on serious vascular events but a 19% reduction in cardiovascular death.[1]
On the omega-6 to omega-3 ratio specifically, NIH's Office of Dietary Supplements notes that experts disagree on whether such a ratio is even a meaningful way to characterize intake, and that no optimal ratio has been established.[1]
On dysmenorrhea, the 2022 meta-analysis pooling randomized controlled trials found omega-3 supplementation reduced primary dysmenorrhea severity, with the daily dose and participants' age both showing a significant effect on the size of that reduction in meta-regression.[2]
On mechanism, omega-3's broader anti-inflammatory reputation is tied to two pathways: blocking the enzyme that turns arachidonic acid into inflammatory eicosanoids, and separately feeding production of resolvins, maresins, lipoxins, and protectins, molecules involved in resolving inflammation rather than triggering it. Beyond hypertriglyceridemia treatment, StatPearls notes that many studies with conflicting data challenge current understanding of exactly how far those effects extend clinically.[3]
