Definition
Total cholesterol is the combined amount of cholesterol in your blood, counting the cholesterol carried by every type of lipoprotein particle, including HDL, LDL, and VLDL.[3] It's measured from a blood draw as part of a standard lipid panel and usually reported in milligrams per deciliter (mg/dL).[2]

What Total Cholesterol Measures
Cholesterol itself isn't a lab artifact, it's a working molecule. It's a structural component of every cell membrane, and it's the raw material your body uses to make steroid hormones, vitamin D, and bile acids.[1] All steroid hormones are derived from cholesterol, including the estrogens and progestins made in the ovaries and the cortisol made in the adrenal cortex.[1] So the same molecule this test counts is the starting point for estradiol, progesterone, and cortisol.
Cholesterol doesn't dissolve in blood on its own. It travels packed inside lipoprotein particles, and the "total" in total cholesterol means the sum across all of them: the cholesterol carried by HDL, by LDL, and by VLDL.[3] That's why a lipid panel breaks the total down into its parts rather than stopping at one number.
How It's Measured
Total cholesterol comes from a standard blood draw and is one of the four core values on a lipid panel, next to HDL cholesterol, LDL cholesterol, and triglycerides.[2] The values are related mathematically too. The classic Friedewald equation estimates LDL cholesterol by subtracting HDL cholesterol and a triglyceride-based VLDL estimate from the total.[2]
Fasting practice has shifted. Lipid panels were traditionally drawn after a 9 to 12 hour fast, which is why they're often scheduled in the morning.[3] Non-fasting panels are now common because they're simpler and more accessible for patients, though fasting is still preferred in some situations, so the instructions that come with your specific test are the ones to follow.[2]
Why People Track Total Cholesterol
Cholesterol isn't a static number, and in women it moves with reproductive hormones. In the BioCycle study, which measured lipids repeatedly across the menstrual cycles of 259 healthy women, total cholesterol was lower in the luteal phase than in the follicular phase, and levels varied inversely with each woman's own estradiol.[4] Lipids shift across the menopause transition too. In the SWAN cohort, total cholesterol showed a distinct increase within the one-year window around the final menstrual period, a pattern the authors tied to menopause itself rather than to aging alone.[5] A dated cholesterol result carries more context when you know where in that hormonal timeline it was drawn.

What the Research Shows
Cholesterol's job as the precursor for all steroid hormones, the backbone of bile acids, and a building block of cell membranes is textbook biochemistry, and it's what makes this molecule relevant well beyond the lipid panel.[1]
The BioCycle study followed 259 healthy premenopausal women across two menstrual cycles with up to eight blood draws per cycle. Total and LDL cholesterol were lower during the luteal phase than the follicular phase, and estradiol was inversely associated with total cholesterol across the cycle.[4]
In SWAN, a multi-ethnic cohort of midlife women followed through the menopause transition, total cholesterol, LDL cholesterol, and apolipoprotein B all showed substantial increases within the year before and after the final menstrual period, while most other measures changed on a steady line consistent with chronological aging.[5]
