Definition

Lipoprotein(a), or Lp(a), is a cholesterol-carrying particle in blood. It's built like an LDL particle with one extra piece: a protein called apolipoprotein(a) bonded to its surface.[1] How much Lp(a) a person makes is largely set by their genes,[2] and it's measured with a standard blood draw on extended lipid panels.

What Lp(a) Measures

An Lp(a) particle starts with the same basic build as an LDL particle: a core of cholesterol wrapped in a protein called apolipoprotein B-100. What makes it different is a second protein, apolipoprotein(a), that's covalently bound to the apoB-100 backbone through a disulfide bond.[1] That extra protein varies a lot in size from one person to the next, because it's built from a repeating segment called a kringle that different people carry in different numbers.[4]

The level itself is mostly written in your DNA. In a classic 1992 family study, the apo(a) gene alone explained about 91% of the person-to-person variation in plasma Lp(a) concentration.[2] That sets Lp(a) apart from the rest of the lipid panel, where values shift with diet, activity, and medication.

How It's Measured

Lp(a) is measured from a standard venous blood draw. It's not part of a basic cholesterol test, so it usually shows up on extended or advanced lipid panels. Some assays report mass in mg/dL and others report particle count in nmol/L. Because apo(a) size varies so much between people, the two units don't convert cleanly, and researchers have flagged the lack of assay standardization as an obstacle to comparing results across labs.[4] Newer assays are designed to read the same regardless of a person's apo(a) size.[5]

Why People Track Lp(a)

Lp(a) is the lipid value people are most likely to measure once and keep on file. Since the level is largely genetic,[2] a result carries information for years, not weeks. That's why the European Atherosclerosis Society's 2022 consensus statement recommends measuring Lp(a) at least once in adulthood.[3] We're describing that guideline discussion, not telling you to get tested. What the Research Shows

The apo(a) gene was estimated to account for about 91% of the variation in plasma Lp(a) concentration in a 1992 study of families, with the number of kringle repeats explaining most of that share.[2]

A 2017 review in the Journal of the American College of Cardiology describes Lp(a) as an independent, genetic, and likely causal factor in cardiovascular disease, studied at the population level across large cohorts.[5]

Measurement itself is an active research problem. A 2016 review found that apo(a)'s genetically determined size differences, plus inconsistent units and assay validation, still hinder harmonizing Lp(a) results across labs.[4]

What's the difference between Lp(a) and LDL?
