Definition
LDL cholesterol is the cholesterol carried by low-density lipoprotein, the particle that delivers most of the cholesterol circulating in blood to tissues around the body.[1] It's reported on a standard lipid panel in milligrams per deciliter, and labs either measure it directly or estimate it from the panel's other values.[3]

What LDL Cholesterol Measures
Cholesterol doesn't dissolve in blood, so it travels packed inside lipoprotein particles. The HDL entry covers how those particle types differ. LDL, short for low-density lipoprotein, is the main delivery particle. It carries the majority of the cholesterol in circulation and moves it from the liver out to the body's tissues.[1] An LDL cholesterol result reports the mass of cholesterol riding in those particles, not the number of particles themselves. ApoB is the test that counts particles, and that entry covers the distinction. A related particle, lipoprotein(a), gets its own separate test.
How It's Measured
LDL cholesterol comes from a standard lipid panel, a blood test that also reports total cholesterol, HDL cholesterol, and triglycerides.[5] Many labs don't measure it directly. They estimate it from the other three values in the same draw, using an equation Friedewald and colleagues published in 1972.[2] Direct LDL assays exist too, and labs switch to them when the estimate turns unreliable, such as when triglycerides run very high.[3] The same blood can give slightly different LDL numbers depending on which method the lab used.

LDL cholesterol shifts with hormonal life stages, which is why it belongs in a women's health tracker at all. What the Research Shows
LDL carries the majority of the cholesterol in circulation, which is why lipid panels report it on its own line instead of stopping at total cholesterol.[1]
In SWAN data, LDL cholesterol showed a substantial increase within the one-year window before and after the final menstrual period, alongside total cholesterol and apolipoprotein B. The pattern held across ethnic groups, which is what separated it from the slow, steady drift expected from aging alone.[4]
The Friedewald estimate has documented limits, and laboratory references describe the situations where direct measurement replaces it.[3]

Is LDL cholesterol measured or calculated?
Both happen. Many labs estimate LDL cholesterol from the total cholesterol, HDL cholesterol, and triglyceride values measured in the same blood draw, an approach published by Friedewald and colleagues in 1972. Direct LDL assays also exist, and labs use them when the estimate becomes unreliable, such as when triglycerides are very high. Your lab report reflects whichever method your lab used.
