Definition
An ovulation test, or OPK (ovulation predictor kit), is a urine strip that detects the surge of luteinizing hormone that comes before ovulation.[1] Most are lateral-flow tests: the test line darkens as urinary LH crosses the kit's threshold, flagging the days when ovulation is likely close.[2]

What an OPK Measures
An OPK measures luteinizing hormone in urine. LH sits at a low baseline for most of the cycle, then spikes sharply in the LH surge, and ovulation follows the start of that surge by roughly 34 to 36 hours.[4] During a surge, urinary LH runs from about 20 to 100 mIU/mL, and sensitive kits detect concentrations as low as 22 mIU/mL.[1] In research on healthy women, a positive result predicted ovulation within about 48 hours.[1] Our LH surge entry covers the biology behind the spike itself.
How It's Read
The common format is a threshold strip. It turns positive once urinary LH crosses the kit's set concentration, shown as a test line as dark as the control line, and digital versions read the same kind of strip and display the result as a symbol instead of a line to judge. Thresholds matter because kits differ in where they're set. A study comparing thresholds found 25 to 30 mIU/mL predicted ovulation within 24 hours best, with a positive predictive value of 50 to 60% and a negative predictive value of 98%.[2] Surge levels also differ between women, so a fixed threshold that catches one person's surge can miss another's or fire early on a rise that never becomes one.[3]

Daily testing usually starts several days before the earliest expected surge and continues until a positive shows up. The threshold-comparison study recommends beginning LH testing as early as cycle day 7 so an early surge isn't missed, since ovulation day varies widely between cycles.[2] The same study found LH testing alone less specific than LH combined with cervical mucus observation, which is why the two often get logged together.[2]
One thing an OPK can't do is confirm ovulation. What the Research Shows
Threshold kits involve a real trade-off. In the threshold-comparison study, a low cutoff caught more surges but produced more false positives, and the authors landed on 25 to 30 mIU/mL as the best balance for prediction within 24 hours.[2]
Researchers are also probing what qualitative strips miss. A 2022 study of urinary LH quantification found surges with several peaks, cases where a positive reflected a follicle that luteinized without rupturing, and likely missed surges from LH variants the tests don't pick up, and it suggests measuring total urinary LH quantitatively may improve prediction of ovulation time.[3]
