Definition
Iron-deficiency anemia is a form of anemia that develops when the body's iron stores run too low to keep up with hemoglobin production, the protein red blood cells use to carry oxygen.[1] It's the most common cause of anemia worldwide, and it disproportionately affects people who menstruate and people who are pregnant, largely because of iron lost to menstrual bleeding and the added iron demands of pregnancy.[1][3]

What Iron-Deficiency Anemia Is
Researchers describe iron deficiency as a spectrum, not an on-off state. Stored iron declines first, then the iron available for building new red blood cells falls, and iron-deficiency anemia is the later stage where hemoglobin production has dropped enough to show up in the blood itself.[1] Iron deficiency is the most common nutritional deficiency in the world, and the greatest prevalence of iron-deficiency anemia specifically falls on women of reproductive age.[2] A 2023 FIGO statement on the condition describes that burden as disproportionately borne by girls and women of reproductive age, tied mainly to menstrual and pregnancy-related iron loss.[3]
The symptoms researchers report most often are fatigue, shortness of breath on exertion, and paleness, features common to anemia generally.[1] Others show up more specifically alongside iron deficiency: hair thinning, brittle or ridged nails, pica (an urge to chew on ice or other non-food items), and restless legs. In one small study of 35 women with iron-deficiency anemia, 85.3% reported at least one of these non-anemia symptoms, most commonly diffuse hair loss and pica.[2]

What the Research Shows
The Global Burden of Disease Study 2021 found the disability-adjusted life years attributable to iron deficiency rose between 1990 and 2021, with the highest burden concentrated in low socioeconomic areas, and prevalence among adolescent girls and women of reproductive age runs in the 9% to 11% range.[1]
The 2023 FIGO statement puts numbers on the mechanism: heavy menstrual bleeding may affect as many as half of reproductive-age women, and up to half of pregnant women who aren't yet anemic are already iron deficient by the first trimester.[3]
Menstrual blood loss was an independent, measurable determinant of ferritin and hemoglobin levels in a 2024 study of 473 premenopausal blood donors, on par with other established determinants of iron status.[4]
