Definition
Heavy menstrual bleeding is menstrual blood loss that a person experiences as excessive enough to interfere with their physical, social, emotional, or daily life, rather than a single fixed blood-loss volume.[1] Because no exam or blood test reads out "heavy" directly, research and everyday tracking both quantify it indirectly, through self-rated flow, product counts, and pictorial charts.[3]

What "Heavy" Means
For years, heavy menstrual bleeding (also called menorrhagia) was tied to a laboratory volume cutoff, measured with the alkaline hematin method, a lab technique few clinics ever ran outside a research study. The International Federation of Gynecology and Obstetrics dropped that volume cutoff in its 2018 revision of bleeding terminology: heavy menstrual bleeding is now formally defined as excessive blood loss that interferes with a person's physical, social, emotional, or material quality of life, and it's classified as a symptom rather than a standalone diagnosis.[1] The UK's NICE guideline for assessing and managing heavy menstrual bleeding uses that same impact-based definition in clinical practice.[2]
Self-perceived heaviness doesn't line up neatly with measured blood loss either. In one clinic-based study of 226 women who described their own periods as heavy, those who rated their bleeding as heavier did tend to have higher blood loss on direct lab measurement, but the relationship was far from exact, and only about a third had measured losses above the volume threshold researchers once used to define menorrhagia.[4] That gap is part of why the term is now framed around impact rather than a number a person could check their own bleeding against.
How It's Quantified
Outside a research lab, nobody measures blood loss directly. Why People Track Heavy Bleeding
A single heavy day is hard to interpret on its own, since flow already shifts across the days of any one period. A pattern across months, how many heavy days show up each cycle and whether that count is changing, turns a subjective feeling into something you can actually see over time.
Menstruation is also a recurring, measurable form of blood loss, and population research ties it directly to iron status. What the Research Shows
Because heavy menstrual bleeding is defined by impact rather than a fixed volume, prevalence estimates vary enormously depending on how a study measures it. A 2026 systematic review found reported prevalence varies widely from study to study, depending on whether a study used a self-report survey, a clinical quality-of-life definition, or another method entirely.[5]
The FIGO Menstrual Disorders Committee's 2018 revision formally classifies heavy menstrual bleeding as a symptom, not a diagnosis, one that can appear alone or alongside other bleeding-pattern changes like irregular timing.[1]
The pictorial charts used to quantify flow in research are validated against direct measurement. The original PBAC study found that scores built from product counts and soiling correlated with lab-measured blood loss, which is why the chart remains a standard research tool decades later.[3]
