Restless legs describes an urge to move the legs that starts at rest, often with uncomfortable sensations. Moving or stretching eases it, and it tends to be worse in the evening or at night.[1] Researchers estimate restless legs syndrome affects between 5% and 15% of the population.[1]

The defining features in the research literature are consistent: an uncomfortable urge to move the legs while at rest, relief when you move or get up and walk, and worse severity at night.[2] The sensations start at rest and are described as abnormal but non-painful.[1] That daily rhythm is part of the definition itself: studies describe a diurnal pattern in which symptoms worsen at night.[1]

What the Research Shows

At the population level, neuropathology and imaging studies report decreased iron in specific brain regions, including the substantia nigra and thalamus, among people with restless legs syndrome, and an international task force review likewise states that brain iron deficiency has been implicated in the condition's pathophysiology.[1][3] This is a population-level research association, not a diagnostic marker — it does not mean an individual's restless legs indicate their personal iron status.

Several studies also report an association between restless legs and pregnancy. A 2022 narrative review found hormonal fluctuations and iron metabolism were the most-studied hypotheses for that link, and concluded the evidence is still incomplete.[4]
