Serum iron measures the iron circulating in your blood at the moment of the draw, nearly all of it bound to a transport protein called transferrin.[1] It's one of four tests in a standard iron panel, read alongside ferritin, TIBC, and transferrin saturation rather than on its own.[2]

What Serum Iron Measures
Iron doesn't float free in blood. Once absorbed, it's bound to transferrin, the protein that holds iron in a soluble form, keeps it from generating reactive oxygen species, and ferries it into cells that need it.[1] Serum iron counts that circulating, transferrin-bound iron at the moment of the draw. It's a snapshot of iron in transit, not the much larger amount held in storage. Stored iron is what a ferritin test reflects.
How It's Measured
Serum iron comes from a standard venous blood draw and is reported in micrograms per deciliter. It's usually ordered as part of an iron panel with total iron-binding capacity (TIBC) and transferrin saturation, plus ferritin, because each test describes a different part of how the body carries and stores iron.[2] Transferrin saturation is even calculated from it directly: serum iron divided by TIBC.[3] Labs report their own reference interval with each result.
Why Serum Iron Swings Within a Day
Serum iron is the moody member of the iron panel. Published references describe within-day swings of as much as 30%, and day-to-day variation of roughly 25% to 30% in the same person.[3] The pattern isn't even predictable. When researchers sampled healthy adults at morning, noon, and afternoon, the swings were real but morning values beat afternoon values for only about half of the participants.[4]
Recent meals move it too. In a study that fed participants test meals with a known amount of labeled iron, serum iron rose afterward, and the size of the rise closely tracked how much iron was absorbed.[5] That's normal physiology, not a flaw in the test. It's also why serum iron is read next to steadier companions like ferritin and TIBC instead of standing alone.

What the Research Shows
Transferrin's role as the blood's iron carrier is settled biochemistry: it chelates iron, keeps it soluble, and delivers it to cells, which is what makes serum iron a measure of iron in transit.[1]
A 2002 study that sampled 20 healthy adults across the day found real within-day variation in serum iron but no consistent diurnal pattern, and within-day variation matched day-to-day variation.[4]
The rise in serum iron after an iron-containing test meal correlated strongly with measured whole-body iron absorption, direct evidence that recent intake shows up in the number.[5]
