Definition

Total iron-binding capacity (TIBC) measures how much iron the proteins in your blood, mainly transferrin, can carry in total.[1] Transferrin saturation is the percentage of that carrying capacity actually occupied by iron.[1] Both come from a standard blood draw and are usually reported together on an iron panel alongside serum iron and ferritin.[5]

What TIBC & Transferrin Saturation Measure

Both tests are built around one protein. Transferrin is the blood's main iron carrier, a plasma protein that picks up iron and delivers it to tissues around the body.[2] TIBC measures the total amount of iron all that circulating transferrin could bind if every seat were filled. Because transferrin does almost all of the binding, TIBC in practice reflects how much transferrin is in the blood, with a small contribution from other proteins like albumin.[3]

Transferrin saturation answers a different question: how much of that capacity is in use right now. Labs calculate it from serum iron and TIBC and report it as a percentage.[1]

How They're Measured

Both values come from a standard venous blood draw, usually ordered together with serum iron and ferritin as an iron panel.[5] Each lab sets and reports its own reference interval with the result, and those intervals vary by lab and method.

What the Research Shows

Transferrin's role as the principal plasma transport protein for iron is long established and is the basis for both tests.[1][2]

A 2002 analysis in Clinical Chemistry and Laboratory Medicine confirmed that TIBC and measured transferrin concentration correlate well, while noting that TIBC methods can run slightly high because iron also binds other plasma proteins, mainly albumin.[3]

A 2016 review in Advances in Clinical Chemistry describes transferrin saturation as a useful body-iron biomarker precisely because it combines plasma iron with its transport protein in one index, while cautioning that inflammation and other settings limit what any single measurement can say.[4]
