What Period Flow Measures

Period flow describes bleeding volume, which is a separate question from how long a period lasts or how often one arrives. Two people with the same cycle length can have very different flow, and one person's flow usually changes across the days of a single period. In a study of 201 regularly menstruating women, bleeding was heaviest during the first three days of menstruation.[4]

How It's Measured

The research gold standard is the alkaline hematin method, a lab technique that extracts hemoglobin from collected pads and tampons and converts it to an actual blood volume.[1] Nobody does that at home, so studies also use pictorial charts like the PBAC, which score the number of pads or tampons used and how soaked each one is.[2] Everyday tracking works the same way but simpler: you rate each day as light, medium, or heavy, and product counts or a menstrual cup's markings serve as rough proxies for volume.

In a real-world dataset of 612,613 cycles from app users, the mean period length was 4.0 days with a standard deviation of 1.5 days.[3]

A systematic review of measurement methods treats the alkaline hematin technique as the gold standard for quantifying menstrual blood loss, and concludes that pictorial methods strike a good balance between ease of use and validated accuracy.[1] The original PBAC validation study showed that scores built from product counts and soiling correlate with directly measured blood loss.[2]

On the iron side, menstrual blood loss was an independent determinant of hemoglobin and ferritin levels in a cohort of 473 premenopausal blood donors, alongside factors like time since a person's last donation.[5]
