Definition

The follicular phase is the first part of the menstrual cycle, starting on day 1 of the period and ending at ovulation.[1] FSH stimulates a cohort of ovarian follicles to grow, one becomes dominant, and its rising estradiol output eventually peaks and triggers the LH surge that causes ovulation.[3]

What Happens During the Follicular Phase

The follicular phase covers the first half of the menstrual cycle. It starts on day 1 of the period, so menstruation itself falls inside this phase rather than counting as something separate, and it runs until ovulation.[1] Rising FSH from the pituitary gland recruits a cohort of several ovarian follicles and gets them growing. As that cohort develops, one follicle usually pulls ahead of the rest and becomes the dominant follicle, the one that will go on to release an egg, while the others stop developing.[1]

How the Phase Ends

The dominant follicle produces more and more estradiol as it grows. Once estradiol climbs high enough and stays there, it flips from suppressing the pituitary gland to stimulating it, a switch known as positive feedback.[3] That switch triggers the LH surge, a sharp spike in luteinizing hormone that causes the dominant follicle to release its egg. Ovulation marks the end of the follicular phase.

Why Follicular Phase Length Varies

Follicular phase length is the main reason period predictions drift from cycle to cycle. A pooled analysis of three cohort studies, covering more than 3,300 cycles from women without known fertility problems, found the follicular phase averaged 18.5 days but swung widely, with a standard deviation of 6.5 days.[4] The luteal phase, the second half of the cycle after ovulation, averaged 11.7 days with a standard deviation of only 2.8 days, making it far more consistent from one cycle to the next.[4] Because most of that swing sits in the follicular phase, the luteal phase is easier to pin down while the follicular phase is the harder part to predict ahead of time. What the Research Shows

The estradiol-to-LH-surge switch is well characterized: once estradiol reaches a critical level, ovulation follows the start of the LH surge by roughly 34 to 36 hours.[2]

What still isn't fully mapped out is why follicular phase length itself varies so much from person to person and cycle to cycle. The same pooled cohort analysis that measured the phase-length gap notes that variability as an open area of study rather than something with one settled explanation.[4]
