MCV (mean corpuscular volume) is the average size of your red blood cells, reported as part of a complete blood count.[1] It's one of three red cell indices, alongside MCH (average hemoglobin per cell) and MCHC (average hemoglobin concentration per cell), that describe red cells beyond a simple count.[2]

What MCV Measures

MCV measures the average size and volume of the red blood cells in a sample.[1] MCH, mean corpuscular hemoglobin, is the average amount of hemoglobin inside a single red cell, and MCHC, mean corpuscular hemoglobin concentration, is how concentrated that hemoglobin is relative to the cell's size.[2] Together the three are called the red cell indices. A red blood cell count tells you how many cells you have. The indices describe what those cells are like.

All three are averages across millions of cells, so they describe the sample as a whole, not any single cell. Hematology research uses size vocabulary built on MCV, calling cells smaller than typical microcytic and cells larger than typical macrocytic.[1] That grouping is descriptive. What any one person's numbers mean depends on their full labs and history, which is a conversation for their doctor. Iron connects to this picture because developing red cells use it to build hemoglobin, and research links a limited iron supply during red cell production with smaller cells.[4] Iron stores themselves show up on a different test, ferritin.

How It's Measured

The indices don't need their own blood draw. Analyzers calculate them from the direct measurements on a complete blood count: MCV from hematocrit divided by the red blood cell count, MCH from hemoglobin divided by the red cell count, and MCHC from hemoglobin divided by hematocrit.[3][6] All three land on the same CBC report, and each lab prints its own reference interval next to them.

What the Research Shows

Hematology defines MCV as the average size and volume of red blood cells and builds its cell-size vocabulary on it.[1]

Are MCV, MCH, and MCHC separate blood tests?

No. All three come from a standard complete blood count, calculated from the same draw's hematocrit, hemoglobin, and red blood cell count. Labs report them together as the red cell indices.

What's the difference between MCH and MCHC?

MCH is the average amount of hemoglobin inside a single red blood cell. MCHC is that hemoglobin relative to the cell's size, a concentration rather than an amount. Two cells can carry the same amount of hemoglobin and still differ in MCHC if one cell is bigger than the other.
