Definition
Hypothyroidism is a condition in which the thyroid gland doesn't produce enough thyroid hormone to meet the body's needs.[1] In the most common form, falling thyroid hormone triggers the pituitary gland to release more TSH, and in the United States, autoimmune thyroid disease, known as Hashimoto thyroiditis, is the most common underlying cause.[1]

What Hypothyroidism Is
The thyroid is a small gland at the base of the neck that produces thyroid hormone, mainly thyroxine (T4) and triiodothyronine (T3), which influence metabolism throughout the body. In primary hypothyroidism, the most common form, the thyroid gland itself underproduces these hormones, and the drop in circulating thyroid hormone reduces the negative feedback signal that normally keeps the pituitary gland in check, so the pituitary releases more TSH to try to stimulate the thyroid.[1] That's the mechanism our TSH entry covers in more detail. In the United States, autoimmune thyroid disease, known as Hashimoto thyroiditis, is the most common underlying cause of hypothyroidism.[1]
Documented symptoms of hypothyroidism include fatigue, cold intolerance, weight gain, dry skin, hair loss, constipation, muscle aches, voice changes, and depressed mood, and a slowed heart rate is a commonly documented exam finding.[1] None of these symptoms are unique to hypothyroidism on their own, which is part of why researchers rely on lab measurement rather than symptoms alone to describe the condition.

What the Research Shows
The NHANES III population survey found an overall hypothyroidism prevalence of 4.6% in the US population studied (0.3% clinical, 4.3% subclinical), and both mean TSH and the prevalence of thyroid antibodies, markers of autoimmune thyroid disease, were higher in women than in men.[2]
A case-control study of 171 people with hypothyroidism found 23.4% reported menstrual disturbances, mainly oligomenorrhea and heavy menstrual bleeding, compared with 8.4% of 214 people without a thyroid disorder.[4]
