Definition
Free T4 is the unbound, biologically active portion of thyroxine, the thyroid gland's main secreted hormone. Most circulating T4 rides on carrier proteins like thyroxine-binding globulin, and the free fraction is what tissues can actually use. Free T4 is measured with a blood draw, usually alongside TSH, and it's the form most labs now test instead of total T4.[1]

What Free T4 Measures
Thyroxine, or T4, is the thyroid gland's main hormone output. The gland releases roughly four T4 molecules for every T3 molecule it secretes, making T4 the dominant thyroid hormone circulating in the blood.[1]
Most of that T4 doesn't travel free. It rides on carrier proteins, chiefly thyroxine-binding globulin, which binds T4 more tightly than the other transport proteins in blood.[2] Only the small unbound portion, free T4, can leave the bloodstream and act on tissue. That's the basis of the total-versus-free distinction that runs through thyroid testing: a total T4 result counts every bound and unbound molecule, so it moves whenever carrier-protein levels shift, for reasons like pregnancy, oral contraceptives, or liver disease, even when the amount of active hormone hasn't changed. Free T4 doesn't carry that distortion, which is why most labs now measure free thyroid hormone rather than total.[2][3]

Free T4 comes from a standard blood draw, most often ordered together with TSH as part of a thyroid panel.[3] Fasting isn't required.

What the Research Shows
The thyroid gland secretes T4 at roughly four times the rate of T3, making T4 the primary hormone the gland releases directly into circulation.[1]
Thyroxine-binding globulin has a higher binding affinity for T4 than the other carrier proteins in blood, which is why it carries the majority of circulating T4 and why changes in TBG levels move total T4 results without necessarily changing free T4.[2]
Once free T4 reaches tissue, deiodinase enzymes outside the thyroid gland convert it into T3. What's the difference between free T4 and total T4?
Total T4 counts every thyroxine molecule in the blood, including the large majority that's bound to carrier proteins like thyroxine-binding globulin. Free T4 counts only the small unbound fraction, the part that's actually available to enter tissue. Because carrier-protein levels shift with pregnancy, oral contraceptives, and other factors, total T4 can move even when thyroid function hasn't changed, which is part of why free T4 is the more commonly ordered test today.
Why is free T4 usually tested alongside TSH?
TSH and free T4 sit on opposite ends of the same feedback loop. The pituitary releases TSH to signal the thyroid, and the thyroid releases T4 in response, so running both tests together shows how the loop is functioning rather than just one side of it. That's the standard pairing on most thyroid panels.
Does free T4 turn into T3?
Yes. Once free T4 reaches tissue outside the thyroid, enzymes called deiodinases convert much of it into T3, generally considered the more biologically active thyroid hormone. Research estimates the majority of circulating T3 is produced this way, through peripheral conversion, rather than secreted directly by the thyroid gland.
