Definition
HbA1c, or hemoglobin A1c, is the share of hemoglobin, the oxygen-carrying protein in red blood cells, that has glucose attached to it. Glucose binds to hemoglobin slowly and without enzymes, so the result reflects average blood glucose over roughly the past three months, about the lifespan of a red blood cell.[1]

What HbA1c Measures
Glucose circulating in the blood sticks to hemoglobin inside red blood cells. The attachment happens on its own, without an enzyme driving it, and once glucose is bound it stays for the life of that cell. The more glucose in the blood, the more hemoglobin picks it up. Red blood cells live about three months, so the fraction of glycated hemoglobin at any moment sums up the glucose those cells have been bathed in over that whole stretch.[1] That's what makes it a different kind of number from a blood glucose reading, which captures a single moment.
How It's Measured
HbA1c comes from a standard blood draw, and the lab reports it as a percentage of total hemoglobin. You don't need to fast first. Because the result reflects months rather than the current moment, blood can be drawn at any time of day.[2] That's the practical contrast with fasting glucose, which requires an overnight fast because it measures the glucose in your blood right then.
Why People Track HbA1c
HbA1c compresses months of glucose exposure into one number, which is why it shows up on most routine metabolic panels. A fingerstick or CGM shows moments and days. HbA1c fills in the long view behind them, and in a large multi-center study it tracked closely with average glucose measured by continuous monitoring and frequent fingersticks over three months.[3]
The number isn't purely about glucose, though. Because the measurement rides on red blood cells, anything that changes how long those cells survive can shift HbA1c independent of blood sugar. StatPearls lists conditions like sickle cell anemia, thalassemia, anemia, and recent blood transfusion as ones that can alter results this way.[1] A systematic review found iron deficiency was associated with higher HbA1c values with no matching rise in glucose measures.[4] Iron status is its own trackable metric, covered in our ferritin entry. And a study of healthy pregnant women found HbA1c ran lower in early and late pregnancy than in non-pregnant women.[5] These are population-level patterns, not tools for reading your own result. What the Research Shows
The ADAG study followed 507 people across 10 international centers, combining continuous glucose monitoring with frequent fingerstick checks for roughly three months each. HbA1c correlated strongly with measured average glucose across that window, which is the empirical basis for treating it as a months-long summary.[3]
A 2015 systematic review in Diabetologia pooled studies on anemia and red-cell abnormalities and concluded that iron deficiency and iron-deficiency anemia were likely to raise HbA1c values without any concurrent rise in glucose measures.[4] In other words, the same average glucose can produce different HbA1c results in different bodies.
Pregnancy shifts the number too. A Danish study comparing healthy pregnant women with non-pregnant controls found HbA1c was significantly lower in both early and late pregnancy.[5]

Do you have to fast before an HbA1c test?
No. Because HbA1c reflects months of glucose exposure rather than the current moment, the blood draw can happen at any time of day, fasted or not. That's a practical difference from a fasting glucose test, which requires an overnight fast.
How is HbA1c different from a blood glucose reading?
A blood glucose reading is a snapshot of the sugar in your blood at one moment, and it moves with meals, activity, and time of day. HbA1c is a running average. It reflects how much glucose your red blood cells have been exposed to over roughly the past three months, so a single meal doesn't move it.
Can anything besides blood sugar change an HbA1c result?
Yes. Research shows HbA1c depends on red blood cell lifespan as well as glucose. Conditions that change how long red cells survive, such as some anemias or recent blood loss, can shift the result. Iron deficiency has been associated with higher HbA1c without any change in glucose, and studies of healthy pregnant women found lower HbA1c in early and late pregnancy. These are population-level findings, so a doctor is the right person to interpret any individual result.
