Definition

C-peptide is a short protein chain that the pancreas releases alongside insulin. When beta cells make insulin, they split a precursor called proinsulin into one insulin molecule and one C-peptide molecule, so the two enter the blood in equal amounts. A C-peptide test measures it from a lab blood draw.[1]

What C-Peptide Measures

C-peptide starts as part of a bigger molecule. Beta cells in the pancreas first build proinsulin, a single chain that enzymes then cut into two pieces, insulin and C-peptide.[2] The cell stores both in the same secretory vesicles and releases them together, so every insulin molecule that leaves the pancreas comes with one C-peptide molecule.[1] That one-to-one ratio is the whole reason the test exists. Count the C-peptide in a blood sample and you get a read on how much insulin the pancreas made. The insulin entry covers what insulin itself does once it's circulating.

How It's Measured

C-peptide is a lab test. A phlebotomist draws a venous blood sample, often after a fast, and the lab reports the result, commonly in ng/mL.[4] There's no home device for it and no sensation that maps to it. The number only comes from a blood draw.

Why People Track C-Peptide

C-peptide fixes three problems that insulin creates as a measurement. Insulin lasts only 3 to 5 minutes in circulation, while C-peptide lasts 20 to 30, so its level is steadier and easier to catch.[2] The liver removes about half of the insulin the pancreas secretes on the first pass, while C-peptide passes through with negligible hepatic clearance.[2] And in people who inject insulin, standard insulin assays pick up the injected hormone too, while C-peptide reflects only what the pancreas produced.[3]

Together those properties make C-peptide a widely used research marker for the body's own insulin production and beta-cell function.[1] It shows up on metabolic lab panels alongside glucose and insulin, and clinicians sometimes measure it to describe how much insulin a person's pancreas is making when the type of diabetes is unclear.[3]

What the Research Shows

Because the body clears it more slowly, C-peptide circulates at concentrations roughly five times higher than insulin.[3] The kidneys remove about half of it, mostly by breaking it down in the tissue around the renal tubules.[3]

The equal-amounts release is well established. Insulin and C-peptide leave the beta cell in equimolar amounts, which lets researchers use C-peptide concentrations to assess beta-cell function across study populations.[1],[2]

How is a C-peptide test different from an insulin test?
