Definition

Fasting glucose is the concentration of glucose in the blood after a stretch without food, usually at least eight hours. It's measured from a lab blood draw and gives a baseline reading of blood sugar with the effect of recent meals removed. It appears on standard metabolic lab panels.[1]

What Fasting Glucose Measures

Blood glucose moves all day. Eating raises it, and the body's feedback loop brings it back down over roughly two hours. Between meals, insulin and glucagon trade off to hold the supply steady.[2] A fasting glucose draw catches the quiet side of that loop. After at least eight hours without food, the last meal's rise has cleared, so the sample reflects the level the body maintains on its own.[1]

That's the whole point of the fast. It removes the biggest short-term variable, food, from the measurement. A sample taken mid-afternoon says as much about lunch as about baseline physiology. How It's Measured

Fasting glucose is a lab test. After a fast of at least eight hours, with water still allowed, a phlebotomist draws a venous blood sample and a lab measures the glucose in it, reported in mg/dL.[1] Lab analysis of a venous sample is more analytically accurate than a fingerstick meter reading.[3] The same test run without the fast is called random glucose. It measures the same thing at an unscheduled moment, so a recent meal may still be in the number.

What the Research Shows

Fasting glucose is one of the two ingredients in HOMA-IR, and that index moves with the menstrual cycle. The BioCycle Study measured fasting glucose and insulin in 257 healthy women across full cycles and found HOMA-IR rose from 1.35 in the midfollicular phase to 1.59 in the early luteal phase, a small population-level easing of insulin sensitivity after ovulation.[5]
