What Folate Measures
Folate works as a coenzyme in one-carbon metabolism, the set of reactions that builds the purines and pyrimidines cells assemble into DNA and RNA.[1] That makes it a supply line for any tissue where cells divide quickly. Red-cell precursors in the bone marrow are a clear example: when folate runs short, DNA synthesis stalls and the marrow releases oversized, underdeveloped red cells, a pattern called megaloblastic anemia.[1] Folate also helps convert homocysteine to methionine in a reaction that depends on vitamin B12, so a folate shortfall can show up as a higher homocysteine level.[1]
How It's Measured
Folate is measured from a standard blood draw and reported in nanograms per milliliter. Labs run it two ways. Serum folate moves with recent intake, so it reflects what you've eaten in the days before the draw. Red blood cell folate reflects longer-term status, because red cells take up folate as they form and then carry it for their whole lifespan.[1] Each lab reports its own reference interval with the result.
Why People Track Folate
Folate has one of the clearest public-health stories of any lab value. An embryo's neural tube, the structure that becomes the brain and spinal cord, closes in the first weeks of pregnancy, often before a person knows they're pregnant. In the Medical Research Council's 1991 randomized trial, folic acid taken around conception cut the recurrence of neural tube defects by 72% in higher-risk pregnancies.[2] On the strength of that evidence, the WHO recommends folic acid supplements for women from the time they start trying to conceive, and the US Preventive Services Task Force gives the same advice a Grade A for anyone who could become pregnant.[3],[4] That's their recommendation, not ours, and questions about supplements belong with your doctor. The US went a step further in 1998 and required folic acid fortification of enriched grain products, which is why it's in most bread and cereal on American shelves.[5]

What the Research Shows
The MRC Vitamin Study, a randomized trial across 33 centers, found a 72% protective effect of folic acid against neural tube defect recurrence, with 6 affected pregnancies in the folic acid groups against 21 in the groups without it.[2]
After the US fully implemented grain fortification in 1998, studies using a range of methods found the prevalence of neural tube defects fell by 19% to 32%.[5]
Folate deficiency impairs DNA synthesis in dividing cells and can raise plasma homocysteine, which is part of why folate, B12, and homocysteine often get ordered and read together.[1]
