Definition

Vitamin B12, also called cobalamin, is a water-soluble vitamin built around a cobalt atom.[1][5] The body needs it to make DNA and form red blood cells.[2] It's measured with a standard blood draw, reported in picograms per milliliter, and often ordered alongside folate on the same panel.

What Vitamin B12 Does

B12 is built around a metal atom, cobalt, at its core.[1] Cells use it in DNA synthesis, which is why fast-dividing cells like the ones that become red blood cells depend on a steady supply.[1][2] B12 also serves as the cofactor for methionine synthase, the enzyme that recycles homocysteine back into methionine. That reaction is where B12's chemistry meets folate's, since folate donates the methyl group the enzyme transfers.[2]

Absorbing B12 takes an unusual relay. The stomach's parietal cells secrete a glycoprotein called intrinsic factor, which binds B12 and carries it through the gut until receptors in the last section of the small intestine, the terminal ileum, pull the complex in.[3] No other vitamin depends on a dedicated stomach-made escort like this, and it's why B12 absorption involves the stomach even though uptake happens far downstream.[3]

Serum B12 comes from a standard venous blood draw and is usually reported in picograms per milliliter. Each lab prints its own reference interval next to the result, and intervals vary by lab and testing method. Researchers have also documented limits to serum B12 as a stand-alone marker, because much of what it counts is B12 bound to a protein that cells can't take up.[4] Methylmalonic acid and holotranscobalamin are additional markers studied as ways to describe B12 status from other angles.[4]

B12's place in DNA synthesis is well established: it participates in reactions vital for cell division and growth, which is what links the vitamin to red blood cell production.[1][2]

The intrinsic factor mechanism has been mapped in detail, from secretion by gastric parietal cells to receptor-mediated uptake of the B12 complex in the terminal ileum.[3]

A 2016 review of B12 biomarkers concluded that total serum B12 isn't a reliable marker of B12 status when used alone, and described methylmalonic acid and holotranscobalamin as the markers researchers use to sharpen the picture.[4]
