Definition
17-OH progesterone (17-hydroxyprogesterone, or 17-OHP) is a steroid hormone the body makes as an in-between step on the pathway from progesterone toward cortisol.[1] The adrenal glands produce most of it, and the ovaries add more during the reproductive years.[2],[3] Labs measure it from a blood draw, usually in nanograms per deciliter (ng/dL).[2]

What 17-OH Progesterone Measures
Steroid hormones get built in an assembly line. Each station is an enzyme, and 17-OHP is one of the parts moving down the line. An enzyme called P450c17 adds a hydroxyl group to progesterone, and the result is 17-OHP.[1] In the adrenal glands, the next enzyme, 21-hydroxylase, prefers 17-OHP as its raw material and turns it into 11-deoxycortisol, the direct precursor of cortisol.[1] So a 17-OHP result describes how much of this intermediate is circulating at the moment of the draw. The same enzyme machinery also feeds the androgen pathways that produce androstenedione and DHEA-S.[1]
The adrenal glands aren't the only source. During the reproductive years, ovarian theca cells make 17-OHP too.[3] Ovarian output shifts with the cycle. Population-level production data show roughly 0.5 mg per day in the early follicular phase, rising to about 4 mg per day around ovulation and again in the mid-luteal phase, when the corpus luteum is the ovary's active hormone source.[2]
How It's Measured
17-OHP comes from a standard blood draw, usually reported in nanograms per deciliter (ng/dL).[4] Timing conventions matter more here than for many labs. The level follows a daily rhythm: in one early study of healthy adults, evening values ran at only about 40% of morning values.[5] Because of that swing, and because ovarian output shifts across the cycle, labs conventionally schedule the draw for the early morning and, in people who menstruate, in the early follicular phase.[6] That's lab methodology, the same way FSH draws get pinned to a cycle day.
Why People Track 17-OH Progesterone
17-OHP isn't part of a routine hormone panel. It usually shows up on extended workups that map the steroid pathway in more detail, sitting alongside cortisol, DHEA-S, and androstenedione. It also appears in adrenal workups that clinicians order, and reading those results is the clinician's job, not something to interpret on your own.[4]

What the Research Shows
17-OHP moves with the clock. In an early radioimmunoassay study, plasma 17-OHP showed a marked circadian pattern in healthy adults, with 8 p.m. values at roughly 40% of 8 a.m. values.[5]
It moves with the cycle too. Endocrinology references report ovarian 17-OHP production rising from about 0.5 mg per day in the early follicular phase to around 4 mg per day in the preovulatory and mid-luteal phases.[2]
A 2024 study that sampled women across reproductive phases traced the ovarian contribution to its source: in the follicular phase, theca cells did most of the 17-OHP synthesis, while granulosa cells made comparatively little.[3]

Is 17-OH progesterone the same as progesterone?
No. They're two different hormones one step apart on the same pathway. An enzyme converts progesterone into 17-OH progesterone, which the adrenal glands then process further toward cortisol. Progesterone is the hormone the corpus luteum secretes after ovulation, and labs measure the two separately.
Why do labs draw 17-OH progesterone in the morning?
Because the level changes across the day. At the population level, 17-OHP runs highest in the early morning and falls toward evening, following the same daily rhythm as cortisol. Drawing in the morning is a lab convention that keeps results comparable from one draw to the next.
Does 17-OH progesterone change across the menstrual cycle?
Yes. The ovaries contribute to the circulating pool, so production shifts with the cycle. Population-level data show output rising around ovulation and staying higher through the mid-luteal phase than in the early follicular phase. That's why labs often note cycle timing alongside the draw.
