Definition

Alkaline phosphatase (ALP) is an enzyme made in several tissues, mainly the liver and bone, with smaller amounts from the intestine and, during pregnancy, the placenta.[1] A blood ALP result reflects the combined output of those sources. It's measured with a standard blood draw, usually as part of a liver or metabolic panel.[2]

What ALP Measures

ALP isn't a single-organ marker. The enzyme is made in the placenta, intestine, kidney, bone, and liver, and more than 80% of what circulates in blood comes from the liver and bone.[1] A total ALP result is the sum of all those sources at once, which is what makes it different from liver-cell enzymes like ALT and AST.

Each tissue makes its own version of the enzyme, called an isoenzyme. Labs can separate these forms to work out which tissue a given portion of the total came from, though that's a specialized follow-up test rather than part of a routine panel.[1],[3]

How It's Measured

ALP is measured from a standard venous blood draw and reported in units per liter (U/L). It's a routine part of liver panels and the comprehensive metabolic panel (CMP), so most people get an ALP result at a regular checkup without ordering it separately.[2],[4] Labs report their own reference interval with each result, and that interval varies by lab, age, and testing method.

Why People Track ALP

ALP has a women's-health angle that most liver-panel values don't: pregnancy. The placenta makes its own ALP isoenzyme, and in the late third trimester that placental output adds enough to raise total blood ALP as a normal part of pregnancy.[1] A study of healthy pregnant women found the placental form contributing a growing share of total ALP as pregnancy neared term.[5] So the same lab value can read differently before, during, and after a pregnancy, which is exactly the kind of context a dated timeline preserves.

Age matters too. ALP runs naturally higher in childhood and adolescence because growing bone produces the enzyme.[1],[2] Both patterns are population-level physiology, not something to read into any single result.

What the Research Shows

Human ALP exists as several genetically distinct forms, including placental, intestinal, germ-cell, and a tissue-nonspecific type expressed in liver, bone, and kidney.[3]

In healthy pregnant women, the mix of ALP isoenzymes in blood shifts with gestational age. Bone ALP contributed a larger share around weeks 31 to 32, and the placental form contributed relatively more by week 38.[5]

Across the population, ALP levels peak during childhood and puberty, when active bone growth adds bone-derived enzyme to the circulating total.[1]
