Definition

GGT (gamma-glutamyl transferase) is an enzyme concentrated in the liver, kidney, and pancreas that helps cells recycle the antioxidant glutathione.[1] Because liver and bile-duct cells hold a lot of it, a blood GGT level rises when those tissues are stressed, and clinicians use it to help interpret an elevated alkaline phosphatase result.[2]

What GGT Measures

GGT's job is breaking a specific chemical bond in glutathione, the cell's main internal antioxidant, and passing the freed piece to an amino acid or peptide. That reaction lets a cell recycle the building blocks it needs to make more glutathione, which is part of how the body defends itself against oxidative stress and clears out toxins.[3] The enzyme is highly represented in the kidney, liver, and brain, and unlike ALT and AST, which sit inside liver cells, GGT is anchored to the outside of the cell membrane, with its active site facing out.[3] Because so much circulating GGT comes from the liver, a blood test mainly reads what's happening there.[3]

How It's Measured

GGT is measured from a standard venous blood draw and reported in units per liter (U/L). It's usually ordered alongside the rest of a liver panel, next to ALT, AST, alkaline phosphatase, and bilirubin, rather than as a stand-alone test.[1],[4] GGT's specific job on that panel is localizing an elevated alkaline phosphatase result. ALP comes from both liver and bone, but GGT isn't made by bone, so a GGT level that rises along with ALP points toward a liver or bile-duct source, while a normal GGT alongside high ALP points elsewhere.[2] Each lab reports its own reference interval with the result.

Why People Track GGT

GGT is also used to screen for or monitor alcohol use, since alcohol is one of several factors known to raise it.[1] Medications and other conditions can raise it too, and researchers describe GGT as easily induced by these factors, which limits how much a single elevated result can say on its own.[2] That's exactly why it's read alongside the rest of a liver panel instead of by itself, and why a dated series that includes GGT next to ALT, AST, and alkaline phosphatase carries more information than any one value alone.

GGT's role in glutathione metabolism is well established: it catalyzes cleavage of the gamma-glutamyl bond in glutathione and transfers the released group to amino acids or peptides, a reaction that helps cells maintain their internal antioxidant defenses.[3]

Clinical references describe GGT and, where available, 5'-nucleotidase as the standard way to localize an elevated alkaline phosphatase result to the hepatobiliary system rather than bone.[2]
