Definition

Sperm DNA fragmentation refers to single- and double-strand breaks in the DNA packaged inside a sperm's nucleus.[2] It's reported as the DNA Fragmentation Index (DFI), measured using assays such as SCSA, TUNEL, or the Comet assay.[2] Current professional guidance doesn't recommend routine DFI testing in the initial evaluation of an infertile couple.[1][3]

What DFI Measures

DNA fragmentation describes breaks in the DNA strand inside the sperm's nucleus. A single-strand break leaves one strand intact so it can still act as a template for repair, and it can arise from oxidation, alkylation, or ordinary DNA-repair activity going wrong mid-process.[2] A double-strand break is more disruptive, since both strands are cut and the break can lead to rearranged genetic material rather than a clean repair.[2]

Research describes three processes that contribute to fragmentation. Incomplete chromatin packaging happens when the exchange of histones for the proteins that compact sperm DNA doesn't fully finish, leaving breaks unrepaired. Abortive apoptosis happens when a sperm cell that started a natural cell-death process survives without completing it. Oxidative stress happens when reactive oxygen species damage the DNA directly and can trigger apoptotic pathways in the cell.[2] These are described as underlying biological mechanisms, not something inferred from any single result.

How It's Measured

A handful of assay families measure fragmentation in different ways. The Sperm Chromatin Structure Assay (SCSA) uses a fluorescent dye that binds differently to intact versus denatured DNA, so intact chromatin resists denaturation and fluoresces green while fragmented DNA denatures more easily and fluoresces red.[2] TUNEL (terminal deoxynucleotidyl transferase dUTP nick end labeling) works more directly, tagging the broken ends of DNA strands with labeled nucleotides.[2] The Comet assay separates DNA by electrophoresis: intact DNA stays compact under the electric current, while fragmented DNA migrates out into a trailing, comet-shaped pattern, with an alkaline version picking up both single- and double-strand breaks and a neutral version picking up mainly double-strand breaks.[2] Each method measures fragmentation through a different physical signature, which is part of why results from different assays aren't always directly comparable.

DFI comes up specifically because a standard semen analysis doesn't cover it. What the Research Shows

Sperm DNA damage shows up more often in infertile men than in the general population, and research has looked at whether it affects reproductive outcomes.[1] In 2013, the Practice Committee of the American Society for Reproductive Medicine reviewed the evidence on DFI testing and reached a specific, limited conclusion: current methods for assessing sperm DNA integrity "do not reliably predict treatment outcomes and cannot be recommended routinely for clinical use."[1]

That position has held up. The joint male infertility guideline from the American Urological Association and ASRM, most recently amended in 2024, reached the same bottom line through its own evidence review: clinicians "should not recommend sperm DNA fragmentation analysis in the initial evaluation of the infertile couple," a moderate-strength recommendation.[3] Both are statements about test utility, not about the underlying biology. Fragmented DNA is a real, measurable property of a sperm sample, assessed with real assays built on different physical principles. Two evidence reviews, over a decade apart, reached the same conclusion: the testing hasn't been shown to reliably change treatment outcomes, which is why it isn't part of a routine infertility evaluation.[1][3]
