Definition
IVF cycle numbers are the sequence of counts a fertility clinic reports during a retrieval cycle: eggs retrieved, mature (MII) eggs, fertilized (2PN) eggs, day-3 embryos, and day-5/6 blastocysts.[3] Each count is smaller than the one before it, a normal pattern across the population rather than a sign that any one cycle went wrong.[1]

What These Numbers Count
Eggs retrieved is the total number of eggs collected during the retrieval procedure, counted right after the procedure ends. It includes eggs at every stage of maturity, not just the ones ready to fertilize.[3]
Mature eggs, labeled MII (metaphase II), are the eggs that have completed the maturation step needed for fertilization. Only mature eggs move forward to insemination or ICSI.[3]
Fertilized eggs are counted at a fertilization check, done 16 to 18 hours after insemination. An egg is confirmed fertilized when the lab sees two pronuclei, labeled 2PN, along with the release of the second polar body.[3]
Day-3 embryos are fertilized eggs that have kept dividing into a multi-cell, cleavage-stage embryo three days after fertilization. Day-5/6 blastocysts are embryos that have kept developing to the blastocyst stage, with a fluid-filled cavity and two distinct cell types, five to six days after fertilization.[3] Clinics transfer embryos at either the cleavage stage or the blastocyst stage.[3]
Why the Numbers Shrink
The count gets smaller at each step, and that's expected. Not every retrieved egg is mature enough to fertilize. Not every mature egg fertilizes. Not every fertilized egg keeps developing into a day-3 embryo or a day-5 blastocyst. One analysis that traced oocytes all the way from retrieval to live birth across hundreds of retrieval cycles found that only a small fraction of retrieved eggs end up as a baby, describing this drop-off as a normal feature of the biological efficiency of the process, not a marker of anything going wrong in an individual case.[1] The attrition happens at every stage along the funnel, and researchers frame it as population biology: a pattern seen across large groups of cycles, not a verdict on any single one.[1] Embryology labs expect the drop too. Professional consensus guidance sets benchmark rates for each stage, from egg maturity through fertilization and embryo development, because some narrowing at every step is the norm.[4]
Why People Track Them
These are values the clinic reports at each stage of a retrieval cycle, and a lot of people write them down as they come in: retrieval day, the fertilization check the next morning, then updates on day 3 and day 5. It's a natural thing to want to track, but each count is a snapshot from one stage of one cycle. The clinic reading the whole cycle, alongside your protocol and lab conditions, is the one drawing conclusions from it.
Comparing your counts to someone else's, or to your own last cycle, doesn't carry the meaning it seems to. Egg and embryo counts vary widely across patients because ovarian stimulation response, age, and lab conditions all differ from person to person and cycle to cycle.[2] Across a large cross-clinic dataset, the number of eggs retrieved per cycle ranged widely even at the population level, with half of all cycles falling somewhere between 6 and 13 eggs.[2] A raw count on its own, without that context, doesn't tell you much.

Why are there fewer embryos than eggs retrieved?
Each step in the IVF process narrows the count. Not every retrieved egg is mature enough to fertilize, not every mature egg fertilizes, and not every fertilized egg keeps developing into an embryo or blastocyst. That drop-off happens across the whole patient population, not just in individual cycles, and researchers describe it as a normal feature of the process rather than an error at any one step.
Is it normal for the numbers to drop at each step?
Yes. Population-level data on IVF outcomes consistently shows the count shrinking from eggs retrieved to mature eggs to fertilized eggs to embryos to blastocysts. This pattern shows up across large groups of patients and is treated in the research literature as expected biology, not a signal that something has gone wrong.
Should I compare my numbers to other people's?
Comparing raw counts across people, or even across your own cycles, doesn't carry much meaning on its own. Egg and embryo counts vary widely between patients because of differences in ovarian stimulation response, age, and lab conditions, so the same number can reflect very different circumstances. Clinics interpret these counts as part of a full cycle, not as a standalone score.
