Polycystic ovary syndrome (PCOS) is a common hormonal condition, and researchers describe it as the most common endocrine disorder among women of reproductive age worldwide.[1] It's defined by some combination of irregular ovulation, elevated androgen levels, and polycystic ovarian morphology on ultrasound, and clinicians diagnose it using the Rotterdam criteria, which require at least two of those three features.[3]

What PCOS Is

Researchers and clinicians diagnose PCOS using the Rotterdam criteria, which require at least two of three features: infrequent or absent ovulation, hyperandrogenism (elevated androgen levels shown clinically or on a blood test), and polycystic ovarian morphology seen on ultrasound.[3] The framework is built to rule other things out first, since conditions like thyroid disorders can produce a similar picture, and no single feature on its own defines the condition since any one of the three can appear without the other two.[1]

PCOS is also tied to metabolic changes beyond the reproductive system. Insulin resistance is estimated to affect around 70% of women with PCOS, independent of body weight,[1] and researchers describe androgen excess and insulin resistance as reinforcing each other: higher insulin is linked to more androgen output from the ovaries, while androgen excess is linked to lower insulin sensitivity.[1],[4] The World Health Organization notes PCOS carries a longer-term association with insulin resistance, type 2 diabetes, and obesity.[2]

Because PCOS is defined by a combination of features that each move on their own timeline, people who have it often log more than one signal instead of relying on any single symptom. PCOS affects an estimated 10% to 13% of women of reproductive age worldwide by the World Health Organization's estimate, though reported prevalence ranges wider, from about 5% to 26%, depending on which diagnostic criteria a study uses.[2][1] The WHO also estimates that up to 70% of women with PCOS worldwide are undiagnosed.[2]
