Definition

Hirsutism is the growth of coarse, pigmented terminal hair in body areas where hair growth depends on androgens, such as the face, chest, and abdomen.[1] It reflects the interaction between circulating androgen levels and how sensitive each hair follicle is to those hormones.[1]

What Hirsutism Reflects

Hair follicles come in two broad types: small vellus follicles that make fine, nearly invisible hair, and terminal follicles that make thicker, pigmented hair. Androgens can switch a follicle from the first type to the second, and the response depends on the body site because it's built into the follicle itself.[2] Within the skin, follicles convert testosterone into dihydrotestosterone (DHT), a more potent androgen, using the enzyme 5-alpha-reductase. Both the skin and its follicles carry androgen receptors, so follicle sensitivity matters as much as the hormone level in the blood.[1]

What the Research Shows

Hirsutism is common. A consensus statement by the Androgen Excess and PCOS Society puts its prevalence at roughly 10% of women in most populations, with variation across ethnic backgrounds.[4]

The follicle's response to androgens is site-specific. The same hormones that push terminal-hair growth in areas like the face and chest can have no effect, or the opposite effect, on follicles elsewhere on the body.[2]

Hirsutism can also occur when measured androgen levels sit in the usual range. Researchers attribute these cases to heightened follicle sensitivity rather than raised hormones.[1]
