Reverse T3 (rT3)

Definition

Reverse T3, or rT3, is an inactive form of thyroid hormone. When the body breaks down T4, deiodinase enzymes turn it into either active T3 or inactive reverse T3.[1] rT3 rises during serious illness and fasting, and clinical reviews report that measuring it hasn't proven useful in routine care.[4]

What Reverse T3 Measures

The thyroid gland mostly releases T4, a storage form that other tissues then convert into what they need. Enzymes called deiodinases do the converting, and the path splits in two. Type 1 and type 2 deiodinases turn T4 into T3, the active hormone. Type 3 deiodinase strips an iodine atom from the other ring of the molecule and produces reverse T3, which doesn't activate thyroid hormone receptors.[1] A reverse T3 blood test measures how much of that inactive product is circulating.

How It's Measured

rT3 is measured from a standard venous blood draw. It isn't part of a standard thyroid panel. Clinical evaluation of thyroid function starts with TSH, usually alongside free T4, and laboratory-medicine guidance states that rT3 shouldn't be used for thyroid function evaluation.[6]

Why People Track Reverse T3

Major clinical guidance doesn't recommend testing rT3 at all. A 2020 report in Annals of Clinical and Laboratory Science put it flatly: "testing of rT3 is not recommended by any professional practice guidelines."[5] A 2018 review in the Cleveland Clinic Journal of Medicine reached the same place, concluding that measuring rT3 hasn't proven clinically useful and that there's "no rationale for measuring reverse T3 to initiate or to adjust levothyroxine therapy."[4]

Even so, rT3 shows up on some extended thyroid panels and direct-to-consumer lab menus, and the same 2018 review notes that unsubstantiated claims about it circulate widely online.[4] So people often meet this marker on a lab report they've already paid for, without context. rT3 is a real molecule with real physiology behind it, and its value as a test is genuinely debated. What the Research Shows

The body shifts toward rT3 production when energy is scarce. In a classic 1975 fasting study, complete caloric deprivation in nine euthyroid volunteers produced a striking drop in serum T3 with a proportional rise in rT3, which the authors described as T4 metabolism diverting from activating to inactivating pathways.[3]

Serious illness produces a similar pattern. Low T3 with raised rT3 is part of what researchers call euthyroid sick syndrome, or nonthyroidal illness syndrome, seen in critically ill and calorie-deprived patients. Low total T3 appears in roughly 70% of hospitalized patients, without the thyroid gland itself being diseased.[2]

What that pattern hasn't delivered is a useful test. The 2018 Cleveland Clinic Journal of Medicine review found that rT3 levels "can be low, normal, or high regardless of the thyroidal state," so measuring rT3 in sick patients hasn't proven clinically useful for diagnosis.[4]

Is reverse T3 the same as T3?

No. T3 and reverse T3 are built from the same T4 molecule, but different deiodinase enzymes remove an iodine atom from different positions. Removing it from the outer ring produces active T3. Removing it from the inner ring produces reverse T3, which is inactive.

Do doctors routinely test reverse T3?

No. A 2020 report in Annals of Clinical and Laboratory Science states that rT3 testing is not recommended by any professional practice guidelines, and standard thyroid evaluation relies on TSH, usually alongside free T4. A 2018 review in the Cleveland Clinic Journal of Medicine reached a similar conclusion: measuring rT3 hasn't proven clinically useful, and there's no rationale for using it to start or adjust thyroid medication.

Why does reverse T3 rise during illness?
