Definition

Thyroid antibodies are immune proteins that target parts of the thyroid gland. The two measured most often are TPO antibodies, which target thyroid peroxidase, the enzyme the thyroid uses to build hormone, and thyroglobulin antibodies, which target the protein the thyroid uses to make and store hormone. Both come from a blood draw.[1]

What Thyroid Antibodies Measure

Antibodies are proteins the immune system makes, normally to fight things like viruses and bacteria. Thyroid antibodies are the autoimmune version: antibodies directed at the body's own thyroid proteins instead of an outside invader.[1] Each of the two on this page has a specific target. TPO antibodies bind thyroid peroxidase, the enzyme that attaches iodine to build the thyroid hormones T4 and T3.[2] Thyroglobulin antibodies bind thyroglobulin, the large protein that provides the backbone for making thyroid hormone and storing it inside the gland.[2] Together they're the standard lab markers of thyroid autoimmunity, meaning immune activity aimed at the thyroid.[1]

How They're Measured

Both antibodies are measured from a standard blood draw, and labs report them as two separate values. They often appear on a broader thyroid panel next to TSH, free T4, and free T3. A third type, TSH receptor antibodies, exists too but is a separate test with its own target.[3] Clinicians read antibody results in context, alongside the rest of the panel and a person's history, rather than as standalone numbers.[3]

Why People Track Thyroid Antibodies

Thyroid antibodies show up in women's health testing because thyroid autoimmunity leans heavily female. Population research finds detectable antibodies more often in women than men, and autoimmune thyroid conditions follow the same pattern.[4][5] When someone gets a full thyroid panel, the antibody values are the part that speaks to the immune system rather than to hormone output, so people who test repeatedly often want to keep both antibody results together with the rest of their thyroid history.

What the Research Shows

Detectable thyroid antibodies are common. In NHANES III, a survey of more than 17,000 people representing the US population, about 11% had detectable TPO antibodies and about 10% had detectable thyroglobulin antibodies. Both were more prevalent in women than men and became more common with age.[4]

Detectable antibodies and thyroid disease aren't the same thing. Research reviews note that circulating thyroid antibodies also turn up in people with no diagnosed thyroid condition: studies put general-population antibody positivity at roughly 5% to 20%, with about 10% a commonly cited figure.[5]

Of the two, TPO antibodies are the more closely tied to thyroid autoimmunity in the research. The same NHANES analysis found that thyroglobulin antibodies on their own, without TPO antibodies, weren't significantly associated with thyroid disease.[4]
