Definition

Body temperature is the heat level the body maintains, regulated by the hypothalamus, which balances heat production against heat loss through mechanisms like sweating and changes in skin blood flow. Core temperature stays tightly controlled while skin temperature swings more with the environment, and both follow a daily circadian rhythm.[1]

What Body Temperature Measures

Body temperature reflects the balance the hypothalamus keeps between the heat the body makes and the heat it sheds. The thermoregulatory center in the preoptic area of the hypothalamus acts as the body's thermostat: when the body runs warm it triggers sweating, and when it runs cool it constricts skin blood vessels so blood bypasses the skin and holds heat in.[1] The number isn't flat across the day. Temperature follows a circadian rhythm, typically lowest in the early morning around 4 AM and highest in the early evening around 6 PM.[1] A study of healthy adults measured that daily swing at an average of about 0.5°C (0.9°F), with a 6 AM low and a late-afternoon peak.[2]

How It's Measured

Where you measure matters. Core temperature is read internally, while everyday thermometers read peripheral sites: mouth, ear, armpit, or forehead. A meta-analysis of 75 studies covering 8,682 patients found peripheral readings can differ from core measurements by more than half a degree Celsius in either direction, so a reading from one site isn't interchangeable with a reading from another.[3] Wearables add a third layer by sampling skin temperature at the wrist or finger overnight, which is a different measurement again. Why People Track Body Temperature

The famous 98.6°F (37°C) average comes from 19th-century measurements, and modern data has revised it. A 1992 study of healthy adults found a mean oral temperature of 98.2°F (36.8°C),[2] and a 2020 analysis of roughly 190,000 people measured between 1860 and 2017 found mean body temperature in the US has declined steadily by about 0.03°C per birth decade.[4] Those are population facts, not personal targets. What the Research Shows

The population average has moved. The 2020 analysis spanning three US cohorts from 1860 to 2017 found the decline held within each cohort as well as between them, which makes measurement error an unlikely explanation, and the authors point to changes in metabolic rate over that period.[4]

Time of day shapes any single reading. The 1992 study that revised the 98.6°F figure also documented the daily rhythm directly, with a 6 AM low, a 4 to 6 PM high, and an average swing of about 0.5°C between them.[2] That circadian pattern is why basal body temperature charting insists on a reading at the same time every morning, a method covered in the basal body temperature entry.
