A hot flash is a sudden wave of heat, usually felt across the face, neck, and chest, often with sweating and flushed skin.[1] Episodes typically last one to five minutes and are linked to changing estrogen levels and altered temperature regulation in the brain, most often during the menopause transition.[2]

What a Hot Flash Is
A hot flash combines a feeling of intense internal heat with profuse sweating and widened blood vessels near the skin.[2] The leading physiological explanation is a narrowed thermoneutral zone, the range of core body temperature the body tolerates before it starts sweating or shivering. In laboratory studies, that zone measured close to zero in women with frequent hot flashes versus about 0.4°C in women without them, so small rises in core temperature that would normally go unnoticed trigger the full sweating-and-flushing response.[2] Falling estrogen contributes, but estrogen levels alone don't explain it. Plasma estrogen doesn't correlate with how severe symptoms are.[2] Hot flashes and night sweats together are called vasomotor symptoms, and roughly 75% to 80% of women experience them at some point during the menopause transition.[1]

What the Research Shows
The Study of Women's Health Across the Nation (SWAN) followed 1,449 women with frequent vasomotor symptoms and found a median total duration of 7.4 years. Women whose symptoms started before or early in the menopause transition had the longest course, with a median above 11.8 years, while symptoms that began after the final period lasted a median of 3.4 years.[3]
SWAN analyses also report that 60% to 80% of women experience vasomotor symptoms at some point during the transition, with occurrence peaking in late perimenopause and the early postmenopausal years around the final menstrual period.[4]
Laboratory work shows small but measurable rises in core body temperature precede most hot flashes, supporting the narrowed-thermoneutral-zone model of how they're triggered.[2]
