Definition

Hematocrit is the fraction of blood volume taken up by red blood cells, reported as a percentage.[1] It's a standard part of a complete blood count, measured either by spinning a blood sample until the red cells pack together or calculated by automated analyzers from red cell count and average cell size.[2]

What Hematocrit Measures

Blood is red cells suspended in plasma, and hematocrit answers one question: how much of the total volume do the red cells take up?[1] That makes it a different measure from hemoglobin, the oxygen-carrying protein inside those cells, which is reported as a concentration. The two usually rise and fall together, but hematocrit is about volume while hemoglobin is about protein content.

Because it's a fraction of whole blood, hematocrit depends on plasma volume just as much as on the red cells themselves.[2] When plasma volume shifts, the percentage moves even if the body's red cell mass hasn't changed at all.

How It's Measured

The classic method is direct: a lab draws blood into a thin capillary tube, seals it, and spins it in a centrifuge until the red cells pack into a column at the bottom. The height of that packed column relative to the whole sample is the hematocrit, which is why older reports call it packed cell volume.[1] Most modern labs skip the spinning. Automated cell counters calculate hematocrit instead by multiplying the red blood cell count by the mean corpuscular volume (MCV), the average size of a single red cell.[2] Same number, built from the parts rather than the whole.[3]

Why People Track Hematocrit

Hematocrit shows up on nearly every routine blood panel, so anyone who gets regular lab work ends up with a series of these values over the years. Pregnancy is the textbook case for why context matters. Plasma volume expands by about 50% over a normal pregnancy, and that expansion outpaces the growth in red cell mass, so hemoglobin concentration, hematocrit, and red cell count all fall.[4] Researchers call this physiologic hemodilution: the blood gets more dilute because there's more plasma, not because anything happened to the red cells. Seeing where a value fell relative to a cycle or a pregnancy is what a longitudinal chart is for.

What the Research Shows

Hematocrit and hemoglobin are both whole-blood measures, so both depend on plasma volume as well as red cell mass. That's a long-established point in clinical laboratory teaching.[2]

In normal pregnancy, plasma volume rises progressively, with most of the roughly 50% increase in place by 34 weeks. Because plasma expands faster than red cell mass, hemoglobin concentration, hematocrit, and red cell count fall while the average size of each red cell stays steady.[4]

The CBC red-cell values are arithmetically linked: MCV can be derived from hematocrit and red cell count, the same relationship analyzers use in reverse to report a calculated hematocrit.[2][3]
