Definition
The erythrocyte sedimentation rate (ESR) measures how far red blood cells settle in a vertical tube of blood over one hour, reported in millimeters per hour.[1] Inflammation makes red cells clump and settle faster, so the ESR works as a general, nonspecific marker of inflammatory activity in the body.[1]

What ESR Measures
The ESR is exactly what its name says. A lab stands a thin tube of your blood upright, waits one hour, and measures in millimeters how far the red cells have sunk from the top.[1] The distance the cells fall is the result.
The speed of that fall depends on what's in the surrounding plasma. During inflammation, the blood carries more acute-phase proteins like fibrinogen, and these proteins encourage red cells to stack into columns called rouleaux. Stacked cells settle faster than single cells, so a higher ESR reflects more of that protein-driven clumping.[1] That makes it an indirect measure. The test doesn't detect any inflammatory molecule itself, it watches what those molecules do to red cells in a tube.[2]
How It's Measured
The result comes from a standard blood draw, and the reference method is the Westergren method: a standardized upright tube, blood mixed with an anticoagulant, and a one-hour wait, with the result reported in mm/hr.[1] It's one of medicine's oldest lab tests. Polish physician Edmund Biernacki refined its clinical use in the late 1800s, and Alvar Westergren defined the method in 1921 that labs still follow today.[1],[4]
Why People Track ESR
ESR and C-reactive protein are the two blood markers most often used to follow inflammation, and they move on very different clocks. CRP doubles and decays in about six hours and peaks in under two days, while an elevated ESR can take weeks or longer to come back down after its trigger resolves.[2] So CRP behaves like a fast gauge and ESR like a slow one, which is why the two are often measured together. The honest thing to know about the ESR is that it's nonspecific. Research reviews note that it lacks both sensitivity and specificity as a screening tool, meaning a shifted value says something may be going on without saying what.[1],[3] Plenty of ordinary factors also move it at the population level. Values tend to run higher in women than men, rise with age, and rise during pregnancy.[1],[5] That's why context, and a trend over time, matters more for this number than any single reading.

What the Research Shows
The Westergren method has held up as the reference standard for about a century, endorsed by the International Committee for Standardization in Haematology and still based on Westergren's original description.[1]
A 2016 review in WMJ found that ESR and CRP frequently disagree with each other, especially in chronic inflammatory conditions, and concluded that both markers should be read alongside the full clinical picture rather than on their own.[3]
A cross-sectional study of 1,019 healthy pregnant women found that ESR values in pregnancy vary with both gestational age and hemoglobin level, which is part of why pregnancy is a recognized population-level factor behind higher readings.[5]

What does 'sed rate' mean on a lab report?
Sed rate is shorthand for sedimentation rate, the same test as the ESR. Labs may label it ESR, sed rate, or erythrocyte sedimentation rate, and all three refer to how far red blood cells settle in a tube over one hour, reported in mm/hr.
What's the difference between ESR and CRP?
Both are blood markers of inflammation, but they move at different speeds. CRP is a protein the liver makes directly in response to inflammation, and it rises and falls within hours to days. The ESR is an indirect, physical measurement that responds more slowly, and an elevated ESR can take weeks or longer to settle back down.
