Cardio recovery, also called heart rate recovery, is how much a person's heart rate falls in the time right after exercise stops. Apple Watch estimates it automatically from heart rate data collected during and after a workout,[3] and exercise physiologists have studied recovery speed as a marker connected to how quickly the nervous system shifts out of an exercise state.[1]

What Cardio Recovery Measures

Heart rate doesn't drop instantly when exercise ends. During exercise, the sympathetic nervous system, the branch that revs the body up, helps drive heart rate higher. Once effort stops, heart rate falls as the parasympathetic nervous system reasserts itself through the vagus nerve, a process called vagal reactivation. The early portion of that drop, typically measured in the first minute after exercise ends, has traditionally been used as an index of vagal activity, since it's largely a function of how quickly that parasympathetic reactivation happens.[1]

In research settings, heart rate recovery has classically been measured during a graded exercise test: heart rate at peak exertion is compared against heart rate exactly one minute into a recovery period, with the person kept still or walking slowly rather than stopping abruptly.[2] Apple Watch takes a related but automated approach. It measures heart rate continuously through a workout and for several minutes after it ends, using its optical wrist sensor, then estimates Cardio Recovery from that continuous data, surfacing the result in the Health app on iPhone.[3]

What the Research Shows

A landmark 1999 study in the New England Journal of Medicine followed 2,428 adults without a history of heart failure for six years, measuring how much their heart rate fell in the first minute after a graded exercise test. A slower drop in heart rate during that first minute was associated with a higher risk of death over the follow-up period, independent of exercise workload and other factors measured in the study.[2] The authors' working hypothesis was that a delayed fall in heart rate reflects reduced vagal activity, consistent with the broader physiology of how the nervous system disengages from exercise.[2]

A 2015 study in SpringerPlus examined the mechanism more directly, describing early heart rate recovery as a function of parasympathetic reactivation, with sympathetic withdrawal becoming more prominent only later in the recovery period, and noting that a person's resting vagal tone influences how their recovery unfolds.[1]
