Definition

Bone density, or bone mineral density (BMD), is the amount of mineral packed into a given area of bone, usually reported in grams per square centimeter. It reflects how much of the bone's structural mineral is present in that spot, and is most commonly measured using a DEXA scan.[1]

What Bone Density Measures

Bone isn't a fixed, inert structure. It's living tissue that's constantly being broken down and rebuilt through a process called remodeling: osteoclasts clear away old or damaged bone, and osteoblasts lay down new bone matrix in its place. In a healthy adult, formation and resorption are tightly coupled, so total bone mass stays roughly steady after each remodeling cycle.[2] Bone density is a snapshot of where that balance currently sits, how much mineral is packed into a given cross-section of bone at the moment it's measured, not a measure of the bone's size or shape.

How It's Measured

DEXA, short for dual-energy x-ray absorptiometry, is the standard method for measuring bone mineral density and the gold standard clinicians use for it.[3] The scan works by passing two x-ray beams at different energy levels through the body: soft tissue and bone absorb the two energies differently, and that difference is used to calculate bone mass per unit of area, reported in grams per square centimeter.[1] Because the result is an areal measurement rather than a volumetric one, it differs from what a CT scan would report for the same bone.[1]

Remodeling doesn't stay balanced forever. It shifts with age and, for women, shifts noticeably around menopause. Estrogen normally slows how fast bone gets broken down, and as estrogen production declines during the menopause transition, that brake loosens. What the Research Shows

StatPearls' overview of dual-energy x-ray absorptiometry describes the scan as emitting photons at two distinct energy levels, one tuned for soft tissue and one for cortical bone, then combining the two readings into a planar image that assesses bone mass per unit of area.[1]

A StatPearls review focused on osteoporosis in women describes DEXA as the gold-standard method for assessing bone mineral density, while also noting its limits, including that it can't separate trabecular bone from cortical bone the way a volumetric scan can.[3] The same review describes bone loss accelerating in the period around menopause before settling into a slower, more gradual pace in the years that follow.[3]

The Endocrine Society attributes that acceleration largely to declining estrogen, since one of estrogen's normal roles is slowing down how fast bone breaks down.[4]
