Audio exposure covers two related HealthKit measurements: headphone audio exposure, the loudness of sound played through connected headphones, and environmental audio exposure, the ambient sound level an Apple Watch's microphone detects around the wearer. Both are logged in decibels over time rather than as a single reading.[4]

What Audio Exposure Measures

These are two separate metrics that happen to share a unit. Headphone audio exposure is about what's playing directly into your ears. Environmental audio exposure is about the sound level in the room or space around you, independent of whether you're wearing headphones at all. Both are recorded in A-weighted decibels, a scale adjusted to roughly match how sensitive human hearing is at different frequencies.[2]

How It's Measured

Headphone audio exposure is calculated automatically when compatible headphones are connected to an iPhone. Apple notes that measurements are most accurate with Apple or Beats headphones, since those can report volume data directly, while other headphones are estimated from the device's own volume setting.[4] Environmental audio exposure works differently: it comes from the Noise app on an Apple Watch, which uses the Watch's built-in microphone to sample ambient sound levels intermittently throughout the day and sends readings to the Health app.

Hearing health from everyday listening has become a bigger public-health focus as personal audio devices have become constant companions. What the Research Shows

The underlying physiology is well established. Noise-induced hearing loss happens through two related pathways: an intense, immediate sound event can damage the outer hair cells in the cochlea, while chronic exposure over time produces a slower, cumulative process that also degrades outer hair cells and the connecting auditory nerve. Because human sensory hair cells don't regenerate once lost, that kind of damage is permanent, and it's compounded by oxidative stress and reduced blood flow within the inner ear during prolonged exposure.[2]

In 2019, WHO and the International Telecommunication Union jointly published a technical standard for personal audio devices, describing a "sound allowance" system in which a device tracks a listener's cumulative exposure as a percentage of a reference level and surfaces that back to the user as an individualized listening profile, rather than simply capping volume outright.[3] That standard is aimed at how manufacturers design devices and what information they give listeners, not a personal target for any one listener to hit.

What's the difference between headphone audio and environmental audio exposure?
