Definition

Visceral fat is fat stored deep in the abdominal cavity, around organs like the liver, stomach, and intestines, as distinct from subcutaneous fat, which sits just under the skin. It has direct access to the portal venous system and is metabolically active, secreting hormones and inflammatory compounds that subcutaneous fat doesn't release in the same way.[4]

What Visceral Fat Measures

Not all fat behaves the same way in the body. Subcutaneous fat, the kind you can pinch under the skin, is mostly a storage tissue. Visceral fat, wrapped around organs deep in the abdomen, has a direct line into the portal venous system, the blood supply that runs straight to the liver, which subcutaneous fat doesn't have.[4] That direct access is part of why visceral fat is described as metabolically active tissue rather than passive storage. StatPearls notes that visceral adipose tissue secretes active metabolites and pro-inflammatory compounds, including C-reactive protein, leptin, and resistin.[1]

How It's Measured

CT and MRI scans are the imaging methods researchers treat as the reference standard for visceral fat, because they can directly visualize and measure the fat area around abdominal organs. Consumer devices, including smart scales, estimate visceral fat differently: they run a weak electrical current through the body (bioelectrical impedance) and use an algorithm to infer a visceral fat number, rather than imaging the tissue directly.

A 2017 study in Scientific Reports tested how well one common consumer BIA method agreed with CT scans of the same people and found the agreement was poor.[2] The device's error grew as actual visceral fat area increased, a pattern the authors called systematic bias, and it skewed in opposite directions by sex, underestimating visceral fat in women and overestimating it in men.[2] The study's conclusion was direct: this type of BIA "has limited potential to accurately estimate visceral fat in a clinical setting."[2]

Why People Track Visceral Fat

Visceral fat draws research and public interest largely because it behaves differently in the body than fat stored elsewhere, not just because of how much of it someone has. A 2018 study of adults with overweight or obesity found men had higher visceral fat and a higher visceral-to-subcutaneous fat ratio than women, independent of age and BMI, while women carried more of their fat in the lower body.[3] That's a distribution pattern that a single body fat percentage, covered on our Body Fat Percentage page, doesn't capture on its own.

What the Research Shows

The physiology is well documented at the mechanism level. Visceral fat's direct drainage into the portal venous system, and its secretion of adipokines like visfatin, is what separates it functionally from subcutaneous fat, according to StatPearls chapters on obesity pathophysiology and metabolic syndrome.[1][4]

Measuring it precisely outside a scanner is a separate, harder problem. The 2017 CT-comparison study found consumer-grade BIA measurements sit far enough from CT values, with wide limits of agreement, that the authors didn't recommend relying on them for clinical decisions.[2] A smart scale's visceral fat number is best read as a rough, device-specific estimate rather than a measurement of the actual tissue.
